Manage client name changes

Maintain the history of client name changes and search for clients based on their last known name in myEvolv. 

If a client's name changes, the history of the name change displays on the Name Change History sub-form. When searching for a client in the system, the search results include all previous names entered for the client. 

Prerequisite: The Name Change History sub-form is included on the Client Personal Information form.

  1. Go to: Client > Client Information > Personal Information > Demographics.
  2. Choose Select Name and search for and select the client.
  3. In the Name section under Name Change History, enter the Last Name and First Name for the client's previous name. 
  4. Enter the From Date and To Date for when the previous name was used.
  5. Above the Name Change History, change the client's Last Name and First Name and other fields as needed to the new name and information. 
  6. Select Save.
