Manage client name changes
Maintain the history of client name changes and search for clients based on their last known name in myEvolv.
If a client's name changes, the history of the name change displays on the Name Change History sub-form. When searching for a client in the system, the search results include all previous names entered for the client.
Prerequisite: The Name Change History sub-form is included on the Client Personal Information form.
- Go to: Client > Client Information > Personal Information > Demographics.
- Choose Select Name and search for and select the client.
- In the Name section under Name Change History, enter the Last Name and First Name for the client's previous name.
- Enter the From Date and To Date for when the previous name was used.
- Above the Name Change History, change the client's Last Name and First Name and other fields as needed to the new name and information.
- Select Save.
