Generate a Clinical Summary

Use the Clinical Summary form in myEvolv to generate a clinical summary for the referral that may include sections of client information to include in a CCD (Continuity of Care Document).

  1. Go to: Client > Referrals > Clinical Summary > Clinical Summary.
  2. In the Select Client box, search for and select a client. 
  3. In the toolbar select Add New, then select Clinical Summary.
  4. On the Clinical Summary form, enter relevant information and complete the required fields.
  5. In the Summary Generated section
    • The Event defaults to Clinical Summary and Entered With defaults to the program and date when the information was entered in the system.
    • In the Summary Generated For box, select the magnifying glass to select a recipient from the Referral Sources Out dialog.
    • For Generated Date, select another date and time as needed.
    • The Worker of Record defaults to the staff member logged in.
  6. In the Include CCD section
    • Select the Generate CCD check box.
    • In the CCD Purpose field, click the magnifying glass to select an option from the CCD Purpose for Clinical Summary with SDK dialog. Choices available are: Electronic Copy and Office Visit Summary.
    • Optional. Under CCD Sections Included, select the check box for each CCD section to include in the clinical summary.
  7. Click Save.
