When is this report most likely to be helpful?

Services

This report has optional From and Through Dates and offers several report selections to present the information in Aggregate or detailed format.

This report shows (some data points are dependent upon the Report Selection chosen) Case name, Case Type, Initial Date (intake of Family Case), Office Responsible, the Program providing service, the Person Served, the Service Provided, the date, duration, staff who provided service, Activity Type, Encounter With, whether the person was a No Show.

Also, the report shows whether progress notes are attached, the submission status, and if the service has been billed. There are also totals provided: by program and service, by program and case, and by case.

The Date field is a hyperlink to the form which is linked as the ‘Form for Editing – People’ in the Case Event setup. (This is the same form which would be accessible from the individual level chart).

Note: If a Family Case Service is entered with multiple participants, then the service will appear multiple times in the detailed versions of this report; once under each participant.

Service Entries

This report requires From and Through Dates and offers several Report Selections to present the information in Aggregate or detailed format.

This report shows (some data points are dependent upon the Report Selection chosen) Case name, Case Type, Initial Date (intake of Family Case), Office Responsible, the Program providing service, the Person Served, the Service Provided, the date, duration, the date entered (note that this is the data point which is different from the ‘Case Services’ report) staff who provided service, Activity Type, Encounter With, whether the person was a No Show. Also, using icons, the report shows whether there are progress notes attached, the submission status, and whether this service has been billed. There will also be totals provided: by program and service, Totals by program and case, and totals by case.

The Date field is a hyperlink to the form which is linked as the ‘Form for Editing – People’ in the Case Event setup. (This is the same form which would be accessible from the individual level chart).

Progress Notes

This report has optional From and Through Dates and offers several Report Selections to present the information in Aggregate or Detailed format.

This report shows the services entered which have a progress note attached. Note that this report is looking for one of the two progress notes which are setup on the Case Event Setup level.

The report shows (some data points are dependent upon the Report Selection chosen) the case name, worker identified on the progress note, the program providing service, Event name (contact type) Date, duration, Activity Type, Encounter With, Facility, the Date Entered. For each service (in the detailed views), it shows the Case Members Eligible for Service (meaning they are an active member of the Case as of the date of service), whether each attended, the duration of service for each, whether they were a no shoe, and whether there are individual level notes entered. Below all members, the report will list the overall Case note entered for the service.

Progress Notes Entries

This report requires From and Through dates which will apply to the date the Progress notes were entered, not the date of service. In all other respects, this report is the same as the Case Progress Note report

Domain

This report has required From and Through dates and multiple Report Selections to display the information differently, including aggregate vs detailed. There are also optional checkbox parameters to limit results to First Test, Last Test, or only Active Cases.

This report shows (dependent upon the Report Selection chosen)  the Category, program, domain, number of assessments scored, number of clients who took the assessment, the average score, and the average rating (this comes from the field ‘Domain Score Weight’ in the Domain setup). The Detailed report shows all of the above, as well as the client’s name, date of assessment, score, rate, interpretation, and the assessment used to get the score in this domain.

Assessment Result

This report requires From and Through Dates, and offers several Report Selections to present the information in Aggregate or Detailed format.

This report shows the Domains form Family Case Assessments. It will present the Test Name, the Domain, and for every question which feeds the Domain, it will list the number of answers provided to this question overall, the answers, the total who provided each answer, the percentage of respondents who provided each answer. The Detail Report Selection will display the Case name who provided each answer.
