Family Day Care forms

Go to: Resource Families > Resource Families > Family Day Care > Information to view and track information about a family that is a Family Day Care center. On the family profile form in the Other Profiles Information, 'Is Family Day Care Center' must be selected.

These are the Family Day Care forms:

  • General Information - Lists the General Information for the Foster/Adoptive home. Here you can track Vendor Information, Managing Office, Statuses, and Household members.
  • Certification History - the staff responsible enters the event required the certifies the daycare to enroll clients in the facility.
  • Requirements - for the family to enter the daycare, entered by the staff responsible and include a completion date and a description.
  • Placement History - Lists all clients who are or have been placed in the home
  • Questionnaires - details about the clients in the day care - comprehensive.
  • Worker Assignments - Define the worker(s) responsible for profile
  • Workgroup Assignments - Define the workgroup(s) also assigned to this profile.
  • Placement History Report - Same as the previous except in the form of a report.
  • Auto Assigned Workgroups - Lists all automatically assigned workgroups
  • Member Information - Allows you to track individual events for each person in the household
  • Calendar - Read only calendar allowing you to view items completed and upcoming for the profile. Navigate this calendar the same as your staff calendar.
  • Scheduled Rules Link - For workflows, this area allows you to view all rules attached to this profile
  • Services Provided Report - Report listing all services provided. This is typically used to troubleshoot the facility placement events to assure workers can place clients in the home.
