Enroll a client in a program

To receive services, a client must be enrolled in a program at your agency.

You can enroll and discharge a client from a program while leaving the client enrolled in the agency.

  1. Go to Client > Client Information > Critical Information > Enrollment Information.
  2. Search for and select a client.
  3. In the Display dropdown select the type of program.
  4. To the right of the agency name, select the ellipses button (Display Menu of Actions) and select New Program Enrollment.
  5. Enter information for Enrollment Information:
    • Agency Placement (required)  Pre-populated with the name of your agency.
    • Program (required)  Search for and select the name of the program in which you're enrolling the client.
    • Start-Date Time (required)  Select the start date and start time of the enrollment.
    • Program ID#  Enter the program ID.
    • Target Discharge  Enter the date you expect the client to be discharged.
    • Opening Reason  Search for and select the reason for the enrollment.
    • Staff Responsible  Pre-populated with your name as the person logged in to myEvolv.
    • Referral  Select the Referral for this Program Enrollment, if available.
    • Actual Discharge Date  Do not enter a discharge date if you are enrolling the client. This is not required at time of enrollment.
    • Cost Center Override  Search for and enter the cost center description to populate the code. The description often corresponds to the program.
      Note: Only select a cost center when different from program setup.
    • Program Unit Override  Search for and enter the program unit description to populate the code.
      Note: Only select when different from program setup.
  6. Enter information for Service Facility Placement:
    • Placement Type (required)  Select the type of placement for this program.
    • Managing Organization  Select the managing organization if available.
    • Service Facility (required)  Select the lookup icon and choose the facility where services will be provided.
    • Unit (required) Select the appropriate unit.
    • Bed  For residential facilities, enter the bed the client is occupying.
    • Grade  For school-based programs, enter the grade in which the client is enrolled.
    • Schedule of Sessions (conditionally required)  When applicable, enter the schedule.
    • Child of a Placed Client  Check this box to indicate the client is the child of a placed client.
    • Living Arrangement Type  Select the living arrangement type when necessary.
  7. Enter information for Program Enrollment Modifier:
    • Modifier  Select a program enrollment modifier when applicable.
    • Effective Date (required when a modifier is selected)  Enter the start date and time of the modifier.
    • Date Received
    • Expiration Date  When the modifier is ended, enter the expiration date.
  8. Enter information for Worker Assignments:
    • Primary Worker (required)  Select the name of the staff member who is the primary worker for this client.
    • Primary Worker's Role (required)  Select the role the primary staff member will use when working with this client.
    • Other Workers (required)  Name and roles of additional staff members who will work with this client.
    • Workgroups (required)
  9. Enter information for Support Group Enrollment:
    • Support Group (required)
    • Unit/Class (required)
    • Schedule (required)
  10. For Additional Information enter Remarks.
  11. Select Save.

Note: If no clients are enrolled in a specific program, then services will appear under Inactive Programs.
