Document termination of parental rights

In myEvolv, document events where parental rights have been terminated.  

Menu Paths
  • Client > Client Information > Personal Information > Adoption/Court Orders
  • Client > Client Information > Personal Information > Legal History
  • Client > Case Management > Service Management > Adoption Legal Activities
  • Referral > Agency Referral > Referral Information > Legal Information
  • Program Referrals > Program Referrals > Referral Information > Legal History
  • Referral Application > Application Data for Providers > Application Information > Legal Information

    Note: In the Client module, the content displayed from the Adoption/Court Orders form set matches the content in the Legal History form set with the exception that the Legal History form set allows you to enter more criminal history information.
  1. Go to one of the menu paths shown above. 
  2. Search for and select a client.
  3. In the taskbar, click New Event and select one of the forms:
    • Father's Parental Rights Terminated
    • Mother's Parental Rights Terminated
  4. In the form, complete any appropriate information. The following fields are required:
    • Date - Click the calendar to select a date.
    • Program Providing Service - Click the magnifying glass to select a program in the Program by Client's Service Track and Service dialog.
    • Facility Providing Service - Click the magnifying glass to select a program in the Provider Sites by Program dialog.
  5. Select Save.
