Discharge and readmit to an agency
You can perform two main actions for the agency in the Critical Information area of the Client module in myEvolv.
- Discharges - Close all program and facility enrollments. This keeps worker and workgroup assignments open, allowing workers to complete information or enter follow-up information.
- Re-Admits - Re-admit a client that was discharged from your agency. The program, Placement Type, Facility, Unit, and Worker Assignment are already entered. This is taken from the Initial Program enrollment done on Intake. This information can be changed if the re-admit information is different.
Discharge a client from an agency
- Go to: Client > Client Information > Critical Information > Enrollment Information.
- Search for and select a client.
- In the Enrollment Information tab, in the Agency line, select the Ellipses button and select Discharge from Agency.
- In the Agency Discharge dialog, complete all appropriate fields. The following fields are required:
- Discharge Date-Time - Select the calendar and select a date.
- Discharge Type - Click the magnifying glass and select an item from the Event Definitions dialog.
- Closing Reason - Click the magnifying glass and select an item from the Closing Reasons Service Track Table dialog.
- Outcome - Click the magnifying glass and select an item from the Outcome Service Track Table dialog.
- Select Save.
Re-admit a client to an agency
After a discharge, you can complete the following steps to readmit a client to an agency.
- From the toolbar, click Client.
- Search for and select a client.
- Go to: Client > Client Information > Critical Information > Enrollment Information.
- In the Enrollment Information tab, in the toolbar, click the Ellipses button and select Re-Admit to Agency.
Note: The Re-Admit to Agency button displays only after a client has been previously discharged from an agency.
- In the Agency Placement dialog, complete any appropriate fields. The following fields are required:
- The Program, Placement Type, Facility, Unit, and Worker Assignment fields are pre-populated with information from the initial program enrollment done on Intake. You can modify information in these fields as necessary.
- Readmission date - Select the Calendar button to select a date.
- Select Save.
