Create a service plan addendum

In myEvolv create an addendum to add and edit information to a client's completed treatment plan.

  1. Go to: Client > Case Management > Plan Development > Plan Addendums.
  2. Search for and select a client.
  3. In the toolbar, select New Event and choose Service Plan Addendum.
  4. In the Service Plan Addendums dialog, complete any relevant fields. The following fields are required:
  • Type - The system automatically populates this field based on the event you selected.
  • Program - Select a program associated with the service event in the Program by Person and Agency dialog.
  • Treatment Plan - Select a treatment plan in the Service Plan Events - Completed dialog.
  • Actual Date/Time - Select a date and time that the treatment plan was created.
  • Completed By - The system automatically populates this information with the user who completed the treatment plan.
  • Facility Providing Service - Select a facility in the Provider Sites by Program dialog.
  1. Select Save.
