Create Fast Track service entries by client

Use Fast Track in myEvolv to enter a service for a client for a particular combination of program, facility, location, and service date.   

Note: Fast Track uses a system default simple service entry form. No changes can be made to the form used. If an agency has a user-defined form attached to the event being documented, and that form has additional and/or required fields, users will need to navigate to Service Entry to enter additional information.


  1. Go to: Taskbar > Fast Track > Fast Track > Fast Track by Client
  2. In the Service Header section, search for and select a client. 
  3. On the Fast Track Entry form, complete the following required fields.
    • Service Header 
      • Staff Assigned to Client - The system automatically populates with the logged in user. You can update this selection.
      • Client - The system automatically populates the service type based on the selection on the previous screen.
      • Program Providing Service - The system automatically populates the program based on the selection on the previous screen.
      • Facility Providing Service - Select the appropriate facility.
      • Completed By - The system automatically populates with the logged in user. You can update this selection.
      • Location - Location where the service was held.
      • Service Date - The date is automatically populated with the current date. You can update this as needed.
  4. In the toolbar, click Get Services.
    Result: The Services section displays applicable services in a grid. The Service Header section is unavailable.
  5. In the row for a service, select the Provided? check box. This enables the remaining items in the row.
  6. Complete the required fields and any optional fields.
    • Service (Required) - Defaults to the service selected
    • Service Authorization (Optional) - Authorization for the service.
    • Start Time (Required) - Time the service began.
    • Duration (Optional) - Length of time for the service.
    • No Show? (Optional) - Indicates client attendance. 
    • Exempt From Billing (Optional) - Exempts the service from billing.
    • Progress Note (Optional) - Click the Edit Memo icon to enter a progress note.
  7. Select Save.

Result: In Case Management (Client > Case Management > Service Entry) the Fast Track service is listed under the program providing the service.
