Complete a DSR service

In myEvolv complete a service in DSR Service Entry.

Prerequisites: 

  • Treatment Plan contains DSR goals/actions.
  • Treatment Plan DSR actions are linked to a program/facility.
  • Client is enrolled in a program. 
  1. Go to: Taskbar > DSR Service Entry > DSR > DSR Service Entry.
  2. Search for and select a client.
  3. In the Date filter, click and select a date from the calendar.
  4. In the Program filter, click the magnifier to search for and select a program.
  5. In the Facility filter, click the magnifier to search for and select a facility.
  6. Select Apply Filters. The search returns all services/goals for the client that should be or can be provided on that day. Only services/goals that have been scheduled from a client's treatment plan are available.
    • The numbers to the right indicate how many times the service has been competed for the period identified in the client's treatment plan. The frequency can be daily, weekly, or monthly. The first number indicates the number of times the goal/service has been completed. The second number indicates how many times the service should be completed for the period. For example, 0/2 indicate the service has been completed zero of the two times required for the period. 
  7. Click the check box to the left of the available service event to expand it and complete the service:
    1. The Start Time field defaults to the current time. Click in the field to enter a different time.
    2. In the End Time field, enter the time the service was ended. The duration is calculated automatically based on the start and end times.
      Note: The Suggested Duration comes from the treatment plan.
    3. Select the Location where the service was provided.
    4. Under Actions, select the check box for the first action. In the fields, enter details about the action taken. Repeat this step for each action listed.
    5. In the Notes field, enter a progress note related to the goal.
  8. Select Save.

Results:

  • The saved service is locked and submitted.
  • The sub-report showing the implementation frequency automatically updates.
  • Though the service has been completed for the day, the service is visible as an option for users to complete an additional service as needed.
View history
  • The View History button allows users to expand and view all services completed for the goal.
  • View history provides users with a quick, simplified view of completed service(s) without having to navigate and select/open them one at a time.
  1. Go to: Client > Case Management > Service Management > Service Entry.
  2. Search for and select a client.
  3. In the row for a service, click View History to see the completed service(s). 
  4. Select Close History.

  1. Click a completed service to open the DSR Service Completion form.
  2. If the service is set up as auto sign/submit, the form is not modifiable.

Result: Documented actions appear on the sub-form under Treatment Plan Link. If more than one action is completed for a goal, all actions are listed.

View a completed service in Service Entry
  1. Go to: Client > Case Management > Service Management > Service Entry.
  2. Search for and select a client.

Result: The completed service appears in the grid.
