Client health and medical reports

In myEvolv, go to: Reports > Clients > Health / Medical to run the client health and medical reports that show medication and diagnosis information.

System Administrator: For clinical and supervisory reports, there is a setting to mask the entire Social Security number or show only the last four digits of the Social Security number. Go to Setup System EvolvCS System Setup > System Settings/Preferences and SSN Preferences.

Diagnoses Information

This report has an optional As Of Date, and the ability to specify a specific Age of client to present, as well as an option to only show Rule Out diagnoses.

This report will list: the client name (which is a link to the Client Information Standalone), general information (DOB, gender, Medicaid #, ID, Case #, CID, IPD, APD, Current Location) Program(s) the person is enrolled in, along with the facility, unit, and worker for each. It will then list the diagnoses on file, including the Priority, Diagnosis, ICD9, ICD10, DSM4, DSM5, Date Diagnosed, and Date entered into myEvolv.

Note that this report will show all diagnoses, even those with end dates.

Medical, Program Oversight

This report is useful for overall review of diagnoses within a caseload, facility, and so on. This is useful for trending as well as current and future planning.

Missing Diagnoses

This report has required From and Through Dates, as well as the option to include clients who are missing Axis IV and V (they may have a primary diagnosis) or those which are missing ICD-10 diagnosis (They may have a diagnosis with ICD-9 codes on file. This was primarily useful in the switch from ICD-9 to ICD-10 in 2014)

This report will present the clients who are missing diagnoses, as defined in the parameters.

This report will list: the client name (which is a link to the Client Information Standalone), general information (DOB, gender, Medicaid #, ID, Case #, CID, IPD, APD, Current Location) Program(s) the person is enrolled in, along with the facility, unit, and worker for each.

If parameters chosen result in pulling clients who have diagnosis on file, but not the correct ones, (such as DSM5), It will then list the diagnoses on file, including the Priority, Diagnosis, ICD9, ICD10, DSM4, DSM5, Date Diagnosed, and Date entered into myEvolv.

Supervisor, QA, Finance

This is useful to ensure complete records, for clinical and financial needs.

Medications

This report, which has optional From and Through dates, as well as several Report Selections to present the information differently, also has optional parameters to identify a specific age of client, whether the medications are used in the eMAR, and whether to display only Active Clients (in this report, ‘active’ means active Agency enrollment. If the person’s enrollment in all programs has ended but their agency enrollment has not been ended, they will still appear in this report.)

This report will list active medications. The three Report Selections are as follows:

Order by: Program/Client Name/Medication: Will show just clients active in program as of end of reporting period. If Client is in more than one program during that period, then medication repeats.

Order by: Facility/Client Name/Medication: Will show just clients active in the facility as of end of reporting period. If client is in more than one facility during that period, then Medication repeats.

Order by: Diagnosis/Client Name/Medication: Will show just clients with diagnosis which is linked to the prescription, not all on file for the client.

 

Medical

This report is useful to monitor medication prescribing trends, or review overall medication picture for a facility or program or diagnosis.

Medication Administration Grid

This report requires identification of a Month and year.

It will show the client name (Which is a link to the Client Information formset member), the medications which should be administered, and a symbol per medication, per day, indicating whether the medication was provided or not. If the medication was provided, but was provided outside the appropriate time band, the time administered will show in the line for the medication. There is a key for these symbols after each grid.

 Medical

This report is useful to review adherence with medication administration 

Patient Lists

This report has optional dates and several Report Selections to present the information differently. This report will show the client name (which is a hyperlink to the Client Information Standalone), as well as (Dependent upon Report Selection) problems, medications, lab tests, allergies, which are on file based on the dates identified. If no dates are identified, the report will show all.  

Medical

This report is useful for gaining an overall picture of the physical health aspects of a group of clients.
