Care Manager program enrollment

In myEvolv, integrate client program enrollments sent from CareManager.

Release consideration: Beginning with the myEvolv NX 10.1.0075 release, a program assignment is not required in CareManager to create a client record in myEvolv NX.

Important: All CareManager system-defined services must be linked to the generic CareManager program, which is the Program Providing Service set up at Agency > Service Locations > Outside Organizations > General Information > SDK Provider.

  • The CareManager program enrollment record sent to myEvolv creates an event with the identical name in myEvolv. If this program enrollment is discontinued in CareManager, then in myEvolv the program enrollment is updated with an end date. 
  • The staff member providing the service on the CareManager program enrollment is the staff member in myEvolv with 'CM' for the Care Manager Staff ID. Only one staff member in myEvolv can be the defaulted staff for CareManager.
  • If a client is discharged in CareManager, then in myEvolv they are discharged from all active program enrollments sent from CareManager. Client program enrollments created in myEvolv are not affected by discharge in CareManager. 
  • In the Program Enrollment record the Program, Facility provider, and one of the Workgroups are populated with the Program Providing Service, Service Facility, and Workgroup for Program values, respectively, for the CareManager tenant (Agency > Service Locations > Outside Organizations > General Information).
  • Use the CareManager Program Enrollment sub report to view a list of CareManager program enrollment event records in myEvolv. The sub report can be placed on any Client module forms that are available for agency modifications, such as a program enrollment form. The sub report includes the enrollment start and end dates, program name, program and facility providing the service, event completion status, event and agency names, staff providing services, and the date the record was created.

CareManager program enrollment in myEvolv  

The program, facility, and one of the workgroups are those associated with the CareManager tenant at Agency > Service Locations > Outside Organizations > General Information.

  1. In myEvolv, navigate to Client > Case Management > Care Manager Integration > Care Manager Program Enrollment.
  2. The Program Name, Actual Date, End Date, and Staff Providing Services values are those sent from the CareManager program enrollment.

CareManager discharge from a program 

  1. In CareManager, navigate to Client > Discharge Client.
  2. Enter the client's last and first names and click Search.
  3. On the client entry that appears, click Discharge.
  4. On the Discharge form, click New Discharge.
  5. The Discharge Date and Discharge Time default to the current date and time. Click in the fields to select a different date and time.
  6. In the Discharged By field, enter all or part of the provider's name and click Enter.
  7. In the Reason for Discharge field, click the drop-down arrow and select the reason.
  8. In the Comments field, enter details about the discharge.
  9. Click Save and Close.
  10. In myEvolv, the program discharge appears at Client > Client Information > Critical Information > Enrollment Information, listed under the CareManager care coordination agency. The program enrollment has an end date and time identical to the discharge date and time in CareManager.
