LA County - Client Treatment Plan

This document is intended for agencies using myEvolv that provide services within the County of Los Angeles, California; specifically for those agencies that utilize the county-driven Client Treatment Plan (CTP) for services, formerly called the “Client Care Coordination Plan.” This guide covers configuration instructions for setting up the Netsmart-supplied CTP, which saves agencies time from designing the plan themselves. 

L.A County Treatment Plan

This area will detail the forms and fields found in the L.A. County Treatment Plan.

Treatment Plan Overview

The LA – Client Treatment Plan is an update to the LA – Client Coordination Care Plan (CCCP). If you are already familiar with the CCCP Plan, the LAC Plan will be relatively the same but with fewer fields.

The treatment structure comprises the following areas as shown in the screenshot below. We will overview each form in more detail. While Netsmart provides the forms below, each form can be modified by the Agency using form designer, meaning you can customize this plan using Netsmart’s as a basis.

  1. Information: This form uses a standard myEvolv treatment plan form and has not been modified for the LAC Tx Plan.

  1. Completed Information: This form uses a standard myEvolv treatment plan form and has not been modified for the LAC Tx Plan.

  1. (Tier Structure) Category of Treatment: This form uses a standard myEvolv treatment plan form and has not been modified for the LAC Tx Plan. However, the plan does use Categories and requires agencies that use this Tx Plan to either utilize their own Treatment Plan categories or Wiley libraries, or both.

  1. (Tier Structure) Client Long Term Goal: This is a modified goal form made specifically for the LAC Tx Plan.

  • Problem Addressed: This pulls any problems on file for this client based on the Category selected in the “Category of Treatment” area. If no problems exist, security can be given to allow staff to add them on the fly while doing the Tx Plan (although it’s typically recommended to have staff identify Problems/Needs prior to Treatment Planning).
  • Client Long Term Goal: This will select from a list of pre-defined goals tied to the Category selected earlier.
  • Goal Statement (use client direct quote): This is a text box where staff can customize the goal statement of the selected Long Term Goal.
  • Staff Responsible: Staff who established this Goal. (Auto-populated by the staff completing the Tx Plan, but can be changed manually.)
  • Status: This is always established on 1st Treatment Plan, but can be changed in review plans to determine if this particular goal needs to be continued, revised, achieved, or ended.
  • Status Date: Date of the Status.
  1. (Tier Structure) Short-term Goal / Objective: This is a modified short-term goal (objective) form made specifically for the LAC Tx Plan.

  • Must be SMART: This is a non-modifiable remarks box that contains required text. The box can be skipped over from a data-entry perspective; it exists for audit reasons or print-out reasons (so that it displays).
  • Objective Number: This will represent the Objective Number for the objective being entered. If more than one objective is entered per Goal, this field would be updated to reflect that Objective number.
  • Short Term Goal / Objective: This will select from a list of pre-defined objectives tied to the Category selected earlier.
  • Objective Statement: This is a text box where staff can customize the objective statement of the selected short-term objective.
  • Staff Responsible: Staff who established this objective. (Auto-populated by the staff completing the Tx Plan, but can be changed manually.)
  • Client was offered a copy of this objective: This check box can be selected if the client was given a copy of the objective.
  • Declined/No Client Signature: This is a text box that can be used to describe why no signature was taken from the client on this objective. A statement is defaulted into the box -- this can be removed, kept, or replaced as needed.
  • Status: This is always established on 1st Treatment Plan, but can be changed in review plans to determine if this particular goal needs to be continued, revised, achieved, or ended.
  • Status Date: Date of the Status.
  1. (Tier Structure) Clinical Intervention: This is a modified short-term goal (objective) form made specifically for the LAC Tx Plan.

  • Intervention Info: This is a non-modifiable remarks box that contains required text. The box can be skipped over from a data-entry perspective; it exists for audit reasons or print-out reasons (so that it displays).
  • Clinical Intervention: This will select from a list of pre-defined interventions tied to the Category selected earlier.
  • Intervention Statement: this is a modifiable text box where staff can customize the intervention statement of the selected Clinical Intervention.
  • Type of Service: This is a look-up table (LUT) that will indicate the type of service identified for this intervention. The LUT values should match what is defined by L.A. County. 
    • This is a LUT that needs to be set up and modified by your agency before Tx Plan use. 
    • The Table can be found at: .
    • The table name is Type of Service and should be filled in per LA County guidelines. Below should be used as a reference only:

  • Client Involvement: Modifiable text box to describe the Client’s Involvement on this Intervention. “Client agrees to participate by:” is default text found in this box, and can be modified by staff.
  • Biological Family Involved: Yes/No checkbox.
  • Other Family Involved: Yes/No checkbox. If selected, this enables the Other Family Description text field.
  • Other Family Description: This remarks field is disabled by default. If the Other Family Involved check box is selected, it enables.
  • Family is Available: Yes/No Checkbox. If selected, this enables the Family agrees to participate field.
  • Client consents to family participation: Yes/No checkbox. If selected, this disables the Client consents to family participation N/A field.
  • Client consents to family participation N/A: Yes/No Checkbox. If selected, this disables the Client consents to family participation field.
  • Family agrees to participate: This field is disabled by default. If the Family is available box is selected, it enables. If selected, this enables the If Family agrees to participate please specify text field.
  • If Family agrees to participate please specify: This text field is disabled by default. It enables if Family agrees to participate is selected. The field is a text field where you enter how a family will participate in the client’s treatment for this intervention.
  • Status: This is always established on 1st Treatment Plan, but can be changed in review plans to determine if this particular goal needs to be continued, revised, achieved, or ended.
  • Status Date: Date of the Status.
  1. Additional Information: This is a modified form designed specifically for LAC Tx Plans. It contains additional plan-level data that is not part of the Tier Structure.

  • Scanned CTP: This is a document attachment field and allows you to scan and attach a paper LAC CTP Plan (if necessary). This option is useful if you do not capture e-signatures and require scanning in the plan to capture signatures.
  • Prefer a language other than English: Yes/No checkbox. Select this box to enable the Other Language field.
  • Plan was Interpreted: Yes/No checkbox field.
  • Other Language: This field is disabled by default. It enables by selecting the Prefer a language other than English. When enabled it allows the user to type in any other language used during the plan creation.
  • MHS Information: This is a non-modifiable remarks box that contains required text. The box can be skipped over from a data-entry perspective; it exists for audit reasons or print-out reasons (so that it displays).
  • Confidential Information: This is a non-modifiable remarks box that contains required text. The box can be skipped from a data-entry perspective; it exists for audit reasons or print-out reasons (so that it displays).
  1. Other Areas, such as Materials, Participants, and Test Link can be enabled or disabled on the Event Properties window of the Tx Plan event. These are defined by your agency.
  2. Signature Page: If the plan is printed (to capture hand signatures), there is room for this as shown below. These signature fields are present on the plan but do not show up when viewing the plan in myEvolv.

L.A. County Treatment Plan Configuration

This area will detail the form and event locations of the LAC Tx Plan, as well as any configuration steps necessary. 

Treatment Plan Forms

All treatment plans in myEvolv are made up of many forms. The LAC Tx Plan contains custom forms at every level. Below demonstrates where all forms are located. This is mainly for reference purposes as the forms and events are already pre-built. The only actual step required to use them (as is) is to link the events to applicable programs.

NOTE: While each agency has the ability to copy and/or modify the existing plan, make any changes with caution as the plan was designed to be compatible with L.A. County acceptance in mind, especially when removing or making certain key L.A. fields invisible. 

  1. Treatment Plan Steps Designer: The entire plan form can be found at Setup > Treatment and Tests > Treatment Plans/Other > Treatment Plan Steps Designer
  • This contains the form linked to the Initial and Review events. You can add more layers (components). 
  1. Form Designer: Two L.A. County forms used in the Tx Plan can be found at Setup > Treatment and Tests > Treatment Plans/Other > Form Designer
  • LA – CTP Additional Information: This form is used in the component “Additional Information.”
  • LA – CTP SIGNATURE_PAGE: This form is used in the component “SIGNATURE_PAGE” to capture hand signatures.

Treatment Plan Events

Two events are provided and linked to the LAC Tx Plan form. The first is the initial event and the second is the review event. 

  1. To locate the events, navigate to Setup > Treatment and Tests > Treatment Plan Events > Initial Plan Events.
  2. The Initial Plan Event can be found under Initial Plan Events and is named CTP (Initial)

  1. Both events can be edited – event properties can be set as usual (as with all events):

Link Plan to Program(s)

The main configuration step with the LAC Tx Plan is linking it to applicable program(s). 

  1. Navigate to Agency Setup > Services Setup > By Program > Plan Setup
  2. Select a program that needs to use this Tx Plan and edit that program. You will see a similar window (but with your program name --- in my example the program is called WCIS Residential). 
  3. Complete the fields as shown below (at a minimum).

  • If you are using myEvolv’s Routing functionality, you can optionally assign a route to each event by linking it to the Routing Workflow Scheme in both locations. Otherwise, keep the Electronically Signed and Can Submit boxes selected.
