Family Case Treatment Plan design considerations
What distinguishes a family case treatment plan from a treatment plan for an individual is that the family case treatment plan covers the family as a whole plus individual family members.
To design a family case treatment plan you need to identify:
- Treatment plan components that apply to the family as a whole, such as area of treatment, goals, and methods.
- Treatment plan components that apply to each family member, such as goals and interventions
- A 'problem addressed' that is associated with the family as a whole and not to individual family members.
- The program that will use the treatment plan
Points to note about creating family case treatment plans in myEvolv NX:
- The treatment plan can be modified.
- The treatment plan includes an end-date option.
- There is an option to add a subreport to the treatment plan.
- There is a submission and approval process (unlike individual treatment plans, there is no need for a third-level signature for approval).
- For treatment plan components you can incorporate forms for family cases and forms for individuals. You also have the option to create your own forms in Forms Designer.
