Set up Post Discharge Follow-Up Events

Post Discharge Follow-Up events contain question prompts to indicate improvement or lack thereof after discharge from the agency or a program.

Menu path

Setup > Event Setup > System – Modifiable > Event Category

View the Available Events

  1. Navigate to: Setup > Event Setup > System – Modifiable > Event Category.
  2. In the toolbar, click Event Category.
  3. Select Post Discharge Follow Ups.
  4. View the events listed in the Events Setup tab. 

Note the events listed. While some modifications can be made (such as indicating that a progress note is included), these are System Events and allowed modifications are minimal.

Provide Access to the Relevant Formset Member

The Post Discharge Follow Ups are maintained here: Client > Client Information > Critical Information > Post Discharge Follow Ups.

  1. To provide access, navigate to: Setup > Security / Access > Navigation Access Setup > Access to Forms.
  2. Provide access to Post Discharge Follow Ups as appropriate. 

Note: Add, edit, and /delete access is determined on the Event level.

Provide Access to the Relevant Events

  1. Navigate to: Setup > Security / Access > Worker Roles > Security for Events.
  2. Scroll down to the Event Group Post Discharge Follow Ups, and provide access to the Events which will be used by the Agency, and to which this role should have access.

Using Workflows to Trigger the Post-Discharge Follow Ups

To improve the experience for the end-user, as well as to improve the likelihood of compliance, it is highly recommended to use workflows to trigger these Post-Discharge follow ups.

Set Up a Workflow

  1. The Post Discharge Follow Up events must be connected to an Event Group in order to be able to be connected to workflows. Navigate to one of the following locations:
  • Setup > User Tools > Event Groups – System > System Event Groups
  • Setup > User Tools > Event Groups – User > Event Groups

Note: Either option is appropriate; the choice is based upon the Agency’s setup choices.

  1. Scroll to the bottom and select the ellipses to connect the Post Discharge Follow Up Events which will be used by the Agency.

Note: In the following example, the System Event Group, Client Generic Events, has been used.

  1. Select the appropriate event(s), such as 06 Month Follow up and 12 Month Follow up, and click Save. 
  2. Navigate to Agency Setup > Workflows Setup > By Type > Client and set up the workflow according to the Agency’s regulations and expectations. An example is provided below for illustrative purposes.

  1. Activate the workflow. In the example above, if an individual is discharged from the identified program, the Primary Worker will be assigned to complete an ’06 Month Follow up’ after 6 months.
