Form Sets
Form sets are the way in which the user navigates through the system within a module. Modules are broken down into sub-modules, then into form sets, which in turn contain form set members or forms. These forms are related by the label of the form set.

- Rules for form sets:
- During a Netsmart upgrade:
- All New form sets are included.
- Existing form set header changes are made.
- Netsmart Supplied Members that already exist are updated in Event Category, Form Family, Default Forms, and Parameters.
- Always replace module and sub-module setup as it is.
- During a Netsmart upgrade:
- In a form set, an Agency can:
- Add items within pre-defined form sets (i.e. Assessments and Risk Assessments).
- Change captions for those form set items that have been pre-defined.
- Change the order of the items within the pre-defined form set.
- In a form set, an Agency cannot:
- Add a form set (i.e. Clinical testing).
- Add, delete, or change the name of a form set.
Example Navigation
The items available in a form set have the following in common:
- Items available for adding come from the same Event Category.
- All forms launched for each item are from the same Form Family.
- All are contained in the Event Group associated with the form set item.
Open a Form Set
- To open a form set, click the + sign to expand it and expose the form set members beneath it.
- Select the form set member you want.
The list of form sets is sorted by module. You can also make changes to form set members in standalone forms in MyEvolv and other areas of the system. Not all form sets contain form set members.
