Family Registration - PM

Register family members into myAvatar PM, or edit existing family members.

  1. Go to: Avatar PM > Client Management > Client Information > Family Registration.
  2. In the Select Family screen, search for the family name in Last Name, First Name format, and click Select.
    • If a match is found, select the family name to edit.
    • If the family is not found, a dialog box displays.
      • Click New Family. It is recommended to enter the family name in Last Name, First Name format for the head of the household.
  3. In the Family Name field, verify the family name is correct and make corrections if necessary.
  4. In the Family's Address - Street field, enter the address.
  5. In the Family's Address - Street 2 field, enter the address.
  6. In the Family's Address - Zip Code field, enter the zip code in the appropriate 5- or 9-digit format.
  7. In the Family's Address - City field, enter the city.
  8. In the Family's Address - State field, select the state.
  9. In the Family Activation Date field, enter the date in MMDDYYYY format, using the calendar icon, or selecting T or Y for today or yesterday.
  10. When finished with both sections, click Submit.
