Enter one of the following options to control the display of the Location field on claims processing forms. This location is the place of service.

  • '0' - disable Location field in all claims processing forms
  • '1' - enable Location field in all claims processing forms
  • '2' - enable Location field in all claims processing forms; send Location to the parent application in all forms 
  • '3' - enable and require Location field in all claims processing forms
  • '4' - enable and require Location field in all claims processing forms; send Location to the parent application
  • '5' - enable Location field in all forms and require the field in the Fast Service Entry and Claim Processing (CMS 1500) forms 
  • '6' - enable Location field in all forms; require Location field in the Fast Service Entry and Claim Processing (CMS 1500) forms; send Location to the parent application
