Treatment Plan Definition

Problem Functional Group Required

Manage treatment plan library information.

  • Customized treatment plan libraries are available in the myAvatar CWS menu.
  • This form works with the Treatment Plan form.
  • The Treatment Plan Library Definition form works with the Treatment Plan Input (Workflow) form.

Create/Edit Libraries

  1. Go to: Avatar CWS > Treatment Planning > Treatment Plan
  2. Select Library.
    Result: The Library Selections screen displays.
  3. Select Add New Library and Return,
    Or
    To edit an existing library, click Select Library. In the Library Selections screen, select the library name and Return.
  4. In the Library Name field enter or edit the name.
  5. If the library is created by the facility: 
    In the Library Origin field enter text to describe the origin (facility, program, staff person).
  6. In the Effective Date Of Library field enter the date that the library should be enabled.
    Note: The library will not be visible in Treatment Plan prior to this date.
  7. In the Active field select: 
    Yes if the library is active,
    Or
    No to deactivate the library.
    Note: If you select No, the library cannot be selected in treatment plan forms but the library can be reactivated.

Treatment Plan Library Filtering

  • The following fields allow a treatment plan library to be filtered in Treatment Plan form, Treatment Plan Library View.
  • These selections control the type and specific filtering available for the library.
  1. In the Library Can Be Filtered On field, select the properties that can be used to filter the library.
    Selecting multiple choices allows the library to be filtered on any of these choices.
  • Age Group
    Use the client's age group as a filter (selected in the Age Group field).
  • Gender
    Use the client's gender as a filter (selected in the Gender field).
  • Selected Programs
    Use programs selected in the Selected Programs field as a filter.​
  1. Click File.

View Library Linking Information

A treatment plan library must be filed before library linking information can display.

  1. Click Select Library.
    The Library Selections screen displays. Select the library and click Return.
    The relationship between items in the library displays. (Library items are defined and linked in the Item Definition section.)
  2. If appropriate, click Print Library.
    The Treatment Plan Library report displays library information.
