Review of Systems

Record a variety of client physical symptoms.

1. Go to Avatar CWS > Assessments > User Defined Assessments > Review of Systems.
2. In the Select Client screen, enter the client ID in the Client Name/ID field, click Select Client Name/ID Search. Click Ok.
   • If the client has multiple episodes, select the client's episode. 
   • The pre-display will appear if a record has been filed for the client and episode. Otherwise, the form will just open.
3. In the Assessing Clinician field, search for the clinician's name by searching for their ID number or in Last Name,First Name format. Select the clinician. 
4. In the Assessing Date field, enter the date by typing in MM/DD/YYYY format, using the calendar icon, or by selecting the T or Y buttons for today or yesterday.
5. In the Assessment Time field, enter the time by typing it in HH:MM AM/PM format or by selecting the Current Time button. 
6. In the Ear, Eye, Nose, and Throat field, select the applicable value(s). 
7. In the Ear, Eye, Nose, and Throat Explanation field, enter explanation information. 
8. In the Respiratory field, select the applicable value(s). 
9. In the Respiratory Explanation field, enter explanation information. 
10. In the Cardiovascular field, select the applicable value(s). 
11. In the Cardiovascular Explanation field, enter explanation information. This field will be enabled by selection of Other Pain. 
12. In the GI and Endocrine field, select the applicable value(s). 
13. In the GI and Endocrine Explanation field, enter explanation information. 
14. In the GU and Reproductive field, select the applicable value(s). 
15. In the GU and Reproductive Explanation field, enter explanation information. 
16. In the HIV Status field, select the applicable value(s). 
17. In the HIV Status Date field, enter the date by typing it in MM/DD/YYYY format, using the calendar icon, or by selecting the T or Y buttons for today or yesterday. 
18. In the STDs field, select the applicable value(s). 
19. In the Multiple Sex Partners field, select Yes or No. 
20. In the Unprotected Intercourse field, select Yes or No. 
21. In the Other High Risk Behavior field, enter other high risk behaviors. 
22. In the Number of Gravida field, enter the number. 
23. In the Number of Para field, enter the number. 
24. In the Number of Aborts/Miscarriages, enter the number. 
25. In the Date of Last Mammogram field, type in the date. 
26. In the MS and Skin field, select the applicable value(s). 
27. In the MS and Skin Explanation field, enter explanation information. 
28. Once finished, click Submit.
