Patient Health Questionnaire - 9

Complete a patient health questionnaire PHQ-9 from a client's responses to record his/her depression severity.

  1. Go to Avatar CWS > Assessments > Product Assessments > Patient Health Questionnaire-9.
  2. In the Select Client screen, enter the client ID, and select the client.
  3. If the client has multiple episodes, select the client's episode, click Ok.
  4. In the Assessment Date field, enter the assessment date.
  5. In the Assessment Status field, select whether this assessment is a Draft or Final version. Once set to Final, the assessment will become view-only.
  6. In the Assessment Practitioner field, enter the staff name, and select.
  7. In the Select Staff to Notify field, select the staff member to notify upon filing.
  8. If not administering:
    1. If the assessment was not completed for the client, search for and select the reason why in the Reason For Not Administering field. Entering a reason in this field will cause the rest of the fields to become disabled.
  9. If administering:
    1. In the following fields, rate the client's current health status:
      • Little interest or pleasure in doing things
      • Feeling down, depressed, or hopeless
      • Trouble falling or staying asleep, or sleeping too much
      • Feeling tired or having little energy
      • Poor appetite or overeating
      • Feeling bad about yourself — or that you are a failure or have let yourself or your family down
      • Trouble concentrating on things, such as reading the newspaper or watching television
      • Moving or speaking so slowly that other people could have noticed? Or the opposite - being so fidgety or restless that you have been moving around a lot more than usual
      • Thoughts that you would be better off dead or of hurting yourself in some way
    2. In the field If you checked off any problems, how difficult have these problems made it for you to do your work, take care of things at home, or get along with other people?, select the level of difficulty these problems pose for the client.
    3. The Total Score field displays the cumulative score for the assessment.
    4. The Depression Evaluation field displays whether the depression screening responses indicate that the client is overall Positive for depression or overall Negative for depression.
  10. Click Submit.
