Mental Status Assessment

This assessment is used to record the presence and extent of a client's mental impairment.

  1. Go to: Avatar CWS > Assessments > Product Assessments > Mental Status Assessment
  2. On the Select Client screen, enter the client ID in the Client Name/ID field, click Select Client Name/ID Search. Click Ok.
    If the client has multiple episodes, select the client's episode, click Ok.
  3. In the Assessment Status field, select whether this assessment is a Draft or Final version.
  4. In the Assessing Date field, enter the assessment date.
  5. In the Assessing Clinician field, enter the assessing clinician's name, click Process Search to select. Click Ok.
  6. In the Assessing Time field, enter the assessment time.
  7. If the assessment was not completed for the client, search for and select the reason why in the Reason For Not Administering field.
  8. Enter evaluative data on the client's mental state for the following categories:
    • Appearance and General Behavior
    • Speech and Language
    • Orientation and Affect
    • Mood and Motor Activity
    • Thought and Perceptual Disorder
    • Intelligence
    • Suicidal and Homicidal Assessment
  9. In the Mental Status Summary, enter any comments related to the assessment as a whole.
  10. Once finished, click Submit.
