Implantable Device List
Search the Implantable Device Federal Database for Unique Device Identifiers (UDIs) and file them to a client in myAvatar .
Avatar CWS > CWS Utilities > Implantable Device List
Prerequisites:
- The myAvatar CareFabric module must be installed in your system to use this form.
- In the Client Search field, search for and select the desired client.
- The Implantable Devices field displays list of all devices currently associated to the client.
- Select whether or not to Exclude Inactive Records from the list.
- In the Unique Device Identifier Search field, search for and select a unique device ID.
- Click File Implantable Device To Client to add this device to the client's list. It will be added to the existing Implantable Devices list.
- Click Submit.
Device Information
- The following fields populate with the appropriate data for the device selected:
- Brand Name
- Version or Model
- Company Name
- Device Description
- What MRI safety information does the labeling contain
- Device required to be labeled as containing natural rubber latex or dry natural rubber?
Production Identifiers
- The following fields populate with the appropriate data for the device selected:
- Batch/Lot Number
- Serial Number
- Expiration Date
- Manufacturing Date
- Donation Identification Number
