Hospitalizations and Surgeries

1. On the Select Client screen, search for and select the client by name or ID.
2. In the Filter by Classification field, select the type of record to filter the results in the Hospital Data grid.
3. The Hospital Data grid shows all saved records.
   • To add a record, click New Row.
   • To edit a record, select the record from the grid. The remaining fields populate with the saved data.
   • To remove a record, select the record from the grid and click Delete Row.
4. In the When field, enter a date or description of the time when the hospitalization or surgery took place.
5. In the Where field, enter the location of the hospitalization or surgery.
6. In the Classification field, select the type of classification for this record. This is the setting that the Filter by Classification field uses to filter the Hospital Data grid records.
7. In the Reason Code field, search for and select the code.
8. In the Notes field, enter any comments to associate with the record.
9. Click Submit.
