Functional Assessment

Record information on a client's disability, and its effects on their academic and functional performance.

  1. Go to: Avatar CWS > Assessments > User Defined Assessments
  2. Select the correct client and episode.
  3. In the Assessing Clinician field, enter the clinician's name.
  4. Click Process Search.
  5. Click Ok.
  6. In the Assessing Date field, enter the assessment date.
  7. Once finished, click Submit.
