'1' makes the Primary Diagnosis Authorized field required on the Service Authorization, Service Authorization (Unlimited Codes) forms.

'2' makes the Principle Diagnosis field required on the following forms:

  • Fast Service Entry
  • Claims Processing (CMS 1500)
  • Claim Processing (UB92)
  • Claim Processing with Override (CMS 1500)
  • Claim Processing with Override (UB92)

Separate multiple values with '&'.

'0' the above fields are not required in the above forms. '0' must be entered alone.
