Control form locators for Paper HCFA-1500 billing by adding the indicated field to the Paper CMS-1500 section of the Guarantor/Program Billing Defaults form.

'1' adds the Form Locator 18 Display Admission/Discharge Date field. This enables the user to specify if the admission/discharge date should display within Hospitalization Dates Related To Current Services (Form Locator 18) on the HCFA-1500.

'0' removes all fields from their respective forms.
