Sex

  • Date of Birth - This value defaults to the Date of Birth filed with the Update Client Data form.

  • Last Four Digits Of Soc. Sec. No. - This value defaults to the last four digits of the client's Social Security Number filed with the Update Client Data form. This value cannot contains spaces, alphabetic characters, or special characters.

  • First Two Letters Of Last Name At Birth - This field defaults to the first two characters of the client's last name filed with the Update Client Data form. The value must be in uppercase alphabetic characters, without spaces and special characters.

  • First Two Letters Of Current Last Name - This field defaults to the first two characters of the client's last name filed with the Update Client Data form. The value must be in uppercase alphabetic characters, without spaces and special characters.
