Adjustment Reason Code is set up in the Dictionary Update form, in the following files:

  • Batch Claim Processing File - data element 355
  • Other Tabled Files - data element 355.

This dictionary has extended data elements:

  • 355 - Adjustment Reason Code
    Select the adjustment reason code.

    Select the default remark code.
  • 358 - Remark Code (2110-LQ-02)
    Select the remark code.
  • 359 - Remark Code Qualifier (2110-LQ-01)
    Select the remark code qualifier.
  • 365 - Effective Date
    Enter the remark code effective date.
  • 366 - Expiration Date
    Enter the remark code end date.
