Service Documentation Section

Associate data filed in the modeled form to a service or appointment in myAvatar CWS.

  • Once a row of data from a Service Documentation Modeled Form is associated with a service or appointment, it can only be edited or deleted using the Service Documentation Corrections form.

Prerequisites:

  • This functionality is added to myAvatar through myAvatar RADplus 2016 Update #117. Please note that Service Documentation is designed for "Document Routing" not "Classic Workflow"
  • The Service Documentation Table field (on the primary Table Definition section) must be set to Yes.
  • For a service documentation table, there must be a Draft/Final (Document Routing) or Draft/Final field defined on the Column Definition section. If the Service Documentation table is filed without this column, you will receive an error message indicating that it must be included.

  1. In the Service Documentation table, click Add New Item.
  2. In the Table Column field, select the field on the modeled form that you want to send data from. The values in this field are derived from the columns defined in the Column Definition section of this form.
  3. In the Service Table Column field, select the field on the progress note that you want to send data to. These values are the standard progress note fields needed to file a service or appointment in a Modeled Service Documentation form.

Example

Several of the Service Table Columns require mapping to another entity. They are indicated in the Mapped-To Entity Database column.

Table Column Column Type Mapped-To Entity Database Mapped-To Entity Dictionary# Mapped-To Entity Dictionary Name

To map the column, go to the Column Definition section, and select Yes in the Does Dictionary Map To A Different Entity field.

Select the entity under Mapped to Entity Database and search for the Mapped-To Entity Dictionary by number or name.
