SQL Table - RADplus_audit_failed_login
Source table for the Failed Login Report form.
Stores an historic record of users who experienced a failed login attempts to myAvatar , forms, or myAvatar ODBC connections.

| application_name | Application type (Avatar PM, CWS, MSO, etc.) |
| login_failure_reason | Reason login failed (invalid system code, invalid user, deactivated user, or invalid password). |
| login_date | Attempted login date. |
| login_time | Attempted login time. |
| type_of_login | Attempted login type (application, form, ODBC, copy form, or move build environment). |
| system_code | Attempted login system code. |
| username | Attempted login username. |
