Create data collection instrument (DCI) forms in Avatar PM and Avatar CWS.
Details
Benefits of creating modeled forms:

ProviderConnect modeled forms display the same data in ProviderConnect, Avatar PM, and Avatar CWS.

Contracting providers can view and manage information remotely without logging into Avatar.

Data collection instrument (DCI) data is stored in the same database as Avatar forms.

Editing dictionary codes in Avatar PM and Avatar CWS changes the associated codes in ProviderConnect.

Modeled forms display at the bottom of the ProviderConnect Client Sidebar menu.

Forms must be modeled in Avatar PM or Avatar CWS.

The administrator must be given access to the View Manage Dynamic Forms and View Dynamic Forms permissions.

The View Dynamic Forms permission allows the user to see every modeled form that has been added to ProviderConnect.
RADplus Modeled forms
The following RADplus forms are used when creating modeled forms:

Envelope Definition
Define the envelope, the main container for SQL tables and forms. -
Table Definition
Define tables to store form data.
When a form is selected, episode-based forms display an episode selection screen.
Non-episode based forms navigate the user directly to the form. -
Form Definition
Define forms to input and work with data. -
Dictionary Update
Define information to populate dictionary fields.
