Sliding Scale Insulin Template Setup

Define sliding scale insulin instruction templates to be used during the creation of Sliding Scale Insulin orders.

  • Sliding scale insulin templates let the facility define the details of a sliding scale insulin dosing regimen in a structured format, including a series of blood glucose levels (ranges) and corresponding insulin doses to be administered.
  • Sliding scale insulin instructions can only be used for pharmacy-type orders that are defined as insulin orders.
  • There can be an unlimited number of sliding scale insulin templates. Some examples of templates:
    • a generic low-dose template
    • a generic high-dose template
    • templates designed for use with particular insulins
    • templates that pre-define preferences for individual physicians

Prerequisites:

  • Avatar eMAR 2007 Enhancement 24 must be loaded on the system to utilize the full Sliding Scale Insulin functionality.
  1. Select Add New Template or Edit Existing Template. Enter the name of the new or existing template.
  2. Click Print All Templates to print a list of all templates on file to be used as a reference when adding and editing templates.
  3. Optionally select Deactivate to deactivate the template. Deactivating a template makes the template unavailable in Order Entry, but does not delete it. The template can be reactivated by deselecting this field.
  4. Enter or change the template description.
  5. Click Sliding Scale Insulin Template Details to display a grid for entering details for the template.
  6. Click New Row at the bottom of the grid.
  7. Enter Blood Glucose From and Up To values.
  8. Optionally enter the number of units to take before breakfast, lunch, dinner, and bedtime.
    You may enter a number with up to one decimal point.
  9. Enter any comments.
  10. Add, delete, and copy rows and copy cells as needed to set up the sliding scales for the template.
  11. Click Save.
  12. Click Submit.
