Generate a report on service code fees.

  1. In the All Service Codes Or Individual Service Code field:

  • Select All to generate a report on all service code fees.
  • Select Individual to generate a report for a specific service code fee (Service Code field).

  1. In the Service Code field, select the code.

  2. Click Process Report. The Service Fee Report details all myAvatar service code fees.
