Review Legal Entity Information - Cal-PM

Edit legal entity information in myAvatar for mental health service facilities owned by corporations, individuals, or counties.

Prerequisite:


  1. Go to: Avatar PM > Client Management > California Required EDI > Review Legal Entity Information
  2. In the Select Entity screen enter the entity, choose Select Entity Name, then OK.
    • The pre-display dialog displays. Select the legal entity record.
    • Select Add or Edit, as appropriate.
  3. Select Review Report to generate a report that details entity information.
  4. The Legal Entity Code and Legal Entity Name fields display legal entity information.
  5. In the Legal Entity Start Date field, enter the start date.
  6. In the Legal Entity End Date field, enter the end date.
  7. In the Legal Entity demographic fields - enter the appropriate information.
  8. In the Ownership Type field, select the type of ownership for the entity.
  9. In the Management Type field, select type of management for the entity.
  10. In the Fed-Taxpayer-ID field, enter the identification number.
  11. In the 'Contact' fields, enter the contact person's information.
  12. In the Contact Comments field, enter comments associated with the legal entity.
  13. In the Transaction Date field, enter the date. If a record is being edited, this field displays the last date an edit was performed.
  14. In the Transaction Time field, enter the time.
  15. In the Transaction ID field, enter the identification, using the format
  16. In the Transaction Code field, select the status of the transaction.
  17. Select Process.
