CSI Service Record Maintenance - Cal-PM

Edit and update CSI service records. Changes made here will be included in the next CSI file compilation. Previous service record data will populate certain fields, which can be edited.

Prerequisites:

  • A CSI service record with Final status must be submitted through the CSI Submission form.

  1. Go to: Avatar PM > Client Management > California Required EDI > CSI Service Record Maintenance
  2. In the Client ID field, enter the search for and select the client by name or ID.
  3. In the Select Previously Submitted File field, select a previously submitted CSI file that contains the CSI service record.
  4. In the Select Service Record To Edit field, select the service contained in the submitted CSI file to edit.
  5. The Record Reference Number field displays the service record reference number.
    Note: This is a unique number created in the previous compile. This reference number identifies the record, and cannot be edited.
  6. In the Medi-Cal Number field, enter the client's Medi-Cal number.
  7. In the Service Function (Mode 5) field, select the service function.
    Note: Mode of service is associated with the treatment program in the Program Maintenance form (Mode Of Service (CSI) field). Only modes of service associated with the client's treatment program can be edited.
  8. In the Service Function (Mode 10) field, select the service function.
  9. In the Service Function (Mode 15) field, select the service function.
  10. In the Units Of Service field, enter the service units.
  11. In the Units Of Time field, enter the time units.
  12. In the Principal MH Diagnosis field, search for and select the diagnosis. 
    Note: Diagnosis information for the client is retrieved from the Diagnosis form, and can be edited.
  13. In the Secondary MH Diagnosis field, enter the diagnosis, select the Search button, and select the corresponding entry.
  14. In the Additional MH Diagnosis fields, enter diagnosis information as appropriate.
  15. In the Special Population field, select the client's special population type of service.
  16. In the Fiscally Responsible County For Client field, select the county.
  17. In the Place Of Service field, select the place of service.
    Note: Place of service is associated with the treatment program in the Program Maintenance form (Place Of Service (CSI) field).
  18. In the 'Evidence-Based Practices / Service Strategies' fields, select the evidenced-based practice or service strategy from the list.
    Note: Evidence-based practice or service strategy is associated with the treatment program in the Program Maintenance form (Evidence-Based Practices / Service Strategies (CSI) field).
  19. In the Trauma field, select the client's trauma status.
  20. In the Client Index Number field, enter the CIN.
    Note: The client index number is assigned to the client in the Financial Eligibility form (Guarantor Selection section).
  21. In the Axis I Diagnosis field, enter the client diagnosis.
  22. In the Axis I Primary field, select Yes if the axis I diagnosis is primary.
  23. In the Additional Axis I Diagnosis field, enter an additional axis I diagnosis.
  24. In the Axis II Diagnosis field, enter the client diagnosis.
  25. In the Axis II Primary field, select Yes if the axis II diagnosis is primary.
  26. In the Additional Axis II Diagnosis field, enter an additional axis II diagnosis.
  27. In the 'General Medical Condition Summary Code' fields, select the code.
    Note: General medical condition summary codes are associated with the client in the Diagnosis form (General Medical Condition Summary Code (CSI) field).
  28. In the 'General Medical Condition Diagnosis' fields, enter the diagnosis.
  29. In the Axis V / GAF Rating field, enter the client’s current GAF rating (Global Assessment Functioning).
  30. In the Substance Abuse / Dependence field, select Yes if the client abuses or is dependent on a substance.
  31. In the Substance Abuse / Dependence Diagnosis field, enter the client's substance diagnosis.
  32. In the County School field, select the county school.
  33. In the District County Code field and District/Site code fields, select the county and site.
  34. Select Submit.
