State Form - State of Illinois
Note - These testing guidelines assume the user is skilled in the use of, at a minimum, the following:
- Admission (Outpatient)
- State of Illinois DHS/DASA Pharmacy System
- State of Illinois DHS/DASA Pharmacy System File Generation
Scenario 1: State of Illinois DHS/DASA Pharmacy System
Specific Setup:
- The test system is enabled for state of Illinois processing.
- Two clients are selected for entry in the 'State of Illinois DHS/DASA Pharmacy System' form ("Client A" & "Client B"),
Steps
- Open the 'State of Illinois DHS/DASA Pharmacy System' form.
- Select "Client A" in 'Client ID'.
- Select the 'Episode'.
- Validate that the following fields are required: 'Patient ID/RIN', 'Medication Type', 'Clinic', and 'Patient Consent'.
- Enter data for the required fields, and desired optional fields. Select 'Yes' in 'Patient Consent'.
- Click [File].
- Click [OK].
- Remain in the form.
- Select "Client B" in 'Client ID'.
- Select the 'Episode'.
- Enter data for the required fields, and desired optional fields. Select 'No' in 'Patient Consent'.
- Click [File].
- Click [OK].
- Close the form.
- Open the 'DHS/DASA Pharmacy System File Generation'.
- Enter the desired date in the 'Monday's Date' field.
- Enter the desired value in the 'Provider Number' field.
- Select the desired values in the 'Clinic' field.
- Enter the desired value in the 'Filename' field.
- Enter the desired value in the 'Filepath' field.
- Click [File].
- Click [OK].
- Click [Generate].
- Click [OK].
- Close the form.
- Open the file that was generated in the 'Filepath' location.
- Validate that "Client A" has one row of data that contains a "Y" in position 351.
- Validate that "Client B" has one row of data that contains an "N" in position 351.
- Close the file.
- Open the 'State of Illinois DHS/DASA Pharmacy System' form.
- Select "Client A" in 'Client ID'.
- Select the 'Episode'.
- Validate that the following fields are required: 'Transaction Type', 'Monday's Date', 'Patient ID/RIN', 'Medication Type', 'Clinic', and 'Patient Consent'.
- Enter data for the required fields, and desired optional fields. Select 'No' in 'Patient Consent'.
- Click [File].
- Click [OK].
- Remain in the form.
- Select "Client B" in 'Client ID'.
- Select the 'Episode'.
- Enter data for the required fields, and desired optional fields. Select 'Yes' in 'Patient Consent'.
- Click [File].
- Click [OK].
- Close the form.
- Open the 'DHS/DASA Pharmacy System File Generation'.
- Enter the desired date in the 'Monday's Date' field.
- Enter the desired value in the 'Provider Number' field.
- Select the desired values in the 'Clinic' field.
- Enter the desired value in the 'Filename' field.
- Enter the desired value in the 'Filepath' field.
- Click [File].
- Click [OK].
- Click [Generate].
- Click [OK].
- Close the form.
- Open the file that was generated in the 'Filepath' location.
- Validate that "Client A" has two rows of data and both rows contain a "N" in position 351.
- Validate that "Client B" has two rows of data and both rows contain a "Y" in position 351.
- Close the file.

Scenario 1: Crisis Episode Client Admission/Discharge Report (PAS 46N) - Field validation
Specific Setup:
- State Form Dictionary Setup:
- Set up a program - Any existing program.
- Select State Form = New York State Client Admission Report (PAS 44N)
- Select State Form Data Element = Program No. (10001)
- State Form Text Value - Desired value. Note the value.
- Admission:
- An existing client is identified, or a new client is created. Note the client id, admission program and admission date.
- Diagnosis:
- Client has a ‘Diagnosis’ record.
- Discharge:
- Client has a ‘Discharge’ record one or two days after the admission date.
Steps
- Open "Crisis Episode Client Admission/Discharge Report (PAS 46N)" for the client.
- Verify the 'First Two Letters Of Last Name At Birth' contains only letters and does not contain any special characters.
- Verify the 'First Two Letters Of Current Last Name' contains only letters and does not contain any special characters.
- Validate that the following fields have been removed from the form: 'NYSID', 'CJ Consent Date', and 'CJ Consent Revoke Date'.
- Validate that the following additional fields have been added to the form: 'Transgender', 'Asian Origin', and 'Pacific Origin'.
- Validate that the following fields have been added to the form: 'Was MAT Offered by this Program During this Treatment Episode?' and 'Was MAT Prescribed by this Program During this Treatment Episode?'.
- If 'No' is selected in 'Was MAT Offered by this Program During this Treatment Episode?', the 'Was MAT Prescribed by this Program During this Treatment Episode?' field is disabled.
- If 'Yes' is selected in 'Was MAT Offered by this Program During this Treatment Episode?', the 'Was MAT Prescribed by this Program During this Treatment Episode?' field is enabled and required,
- If 'No' is selected in 'Was MAT Prescribed by this Program During this Treatment Episode?', the 'Addiction Medication Used field defaults to 'None'.
- If 'Declined' is selected in 'Was MAT Prescribed by this Program During this Treatment Episode?', the 'Addiction Medication Used field defaults to 'None'.
- If 'Yes' is selected in 'Was MAT Prescribed by this Program During this Treatment Episode?', the 'Addiction Medication Used' field will contain no predefined values and user can select items.
- Fill out all required fields, and desired optional fields.
- Click [File Form].
- Click [OK].
- Close the form.
- Open "Crisis Episode Client Admission/Discharge Report (PAS 46N)" for the client.
- Validate that the form displays the submitted data.
Scenario 2: Client Discharge Report (PAS 45N) - NYS PAS Form Submission File Creation
Specific Setup:
- Facility Defaults:
- The 'Output Path On Server For Electronic Files' field is set to the location where the output files are stored. Note the path.
- Program Maintenance: Identify a substance abuse program.
- State Form Dictionary Setup:
- Set up desired program(s).
- Select State Form = New York State Client Admission Report (PAS 44N).
- Select State Form Data Element = Program No. (10001).
- State Form Text Value = desired value. Note the value.
- Client:
- A new client is admitted to the program identified above. Note client's id / admission date.
- The diagnosis record is created for the client.
- The 'Client Admission Report (PAS 44) has been filed.
- The client has been discharged.
Steps
- Open the form "Client Discharge Report (PAS 45N)".
- Select desired client.
- Select the desired episode.
- Validate that the following fields have been added to the form: 'Was MAT Offered by this Program During this Treatment Episode?' and 'Was MAT Prescribed by this Program During this Treatment Episode?'.
- If 'No' is selected in 'Was MAT Offered by this Program During this Treatment Episode?', the 'Was MAT Prescribed by this Program During this Treatment Episode?' field is disabled.
- If 'Yes' is selected in 'Was MAT Offered by this Program During this Treatment Episode?', the 'Was MAT Prescribed by this Program During this Treatment Episode?' field is enabled and required,
- If 'No' is selected in 'Was MAT Prescribed by this Program During this Treatment Episode?', the 'Addiction Medication Used field defaults to 'None'.
- If 'Declined' is selected in 'Was MAT Prescribed by this Program During this Treatment Episode?', the 'Addiction Medication Used' field defaults to 'None'.
- If 'Yes' is selected in 'Was MAT Prescribed by this Program During this Treatment Episode?', the 'Addiction Medication Used' field will contain no predefined values and user can select items.
- Add data to all required fields and to desired optional fields.
- Submit the form.
- Close the form.
- Reopen the "Client Discharge Report (PAS 45N)" form and validate that the data displays correctly.
- Close the form.
- Open the 'NYS PAS Form Submission File Creation' form.
- Populate the 'Provider ID No' with up to 5 digits.
- Set the 'Filter By PRU Number' field to 'No'.
- Select the month of discharge for the client identified above in the 'Month to report' field.
- Set the 'Re-Send Records Previously Sent' field to 'Yes'.
- Click [Create File].
- Verify the system displays all the records for the client for which the 'Client Discharge Report (PAS 45N)' form is filed.
- If desired, review the file created in the Facility Defaults output path and return to the form.
- Populate the 'Provider ID No' with up to 5 digits.
- Set the 'Filter By PRU Number' field to 'Yes'.
- 'Set the 'PRU Selected By' field to 'PRU'.
- Select the desired PRU value.
- Select the month of discharge for the client identified above in the 'Month to report' field.
- Set the 'Re-Send Records Previously Sent' field to 'Yes'.
- Click [Create File].
- Verify the system displays all the records for the client for the selected PRU number and for that program the 'Client Discharge Report (PAS 45N)' form is filed for the client.
- If desired, review the file created in the Facility Defaults output path and return to the form.
- Close the form.
