Progress Notes (Group and Individual) form - Client in context
Note - These testing guidelines assume the user is skilled in the use of, at a minimum, the following:
- Browse Forms

Scenario 1: Validate selected client in context launching a Form from the "Browse Forms" button
Specific Setup:
- Have two active clients on the system [ClientA] and [ClientB]
- User has the "My Clients" widget on their home view
- User has access to the "Progress Note (Group and Individual)" form and any client based form, for example the "Update Client Data" form
Steps
- In the "My Clients" widget, search for and select [ClientA]
- Validate [ClientA] is selected highlighted in the "Recent Clients" list
- Double click on the [ClientA] to open "Chart"
- Validate the name of [ClientA] is displayed in the "Chart" header
- Under the header, click the "Browse Forms" (+) button
- In the "Search Forms" field search for "Progress Note (Group and Individual)" form
- Validate the "Select Client" field is populated with [ClientA]
- Validate the client header reflects information for [ClientA]
- Populate the desired fields on the form
- Click [File Note]
- Validate submission is successful
- In the "Select Client" field, select [ClientB]
- Validate the name of [ClientB] is displayed in the client header section
- Close the form
- At the home view, validate [ClientA] is still selected highlighted in the "Recent Clients" list
- At the home view select and open form "Update Client Data"
- Validate [ClientA] is selected highlighted in the "Recent Clients" list
- Search and open the "Update Client Data" form
- Validate the name of [ClientA] is displayed in the client header section
- Under the header, click the "Browse Forms" (+) button
- In the "Search Forms" field search for "Progress Note (Group and Individual)" form
- Validate the client header reflects information for [ClientA]
- Populate the desired fields on the form
- Click [File Note]
- Validate submission is successful
- In the "Select Client" field, select [ClientB]
- Validate the name of [ClientB] is displayed in the client header section
- Close the form
- At the home view, validate [ClientA] is still selected highlighted in the "Recent Clients" list

- Have any other client based form [FormB] also set up in "Form Designer" with widget [WidgetA] selected in field "Widgets to Display on Form". For this test form "Update Client Data" is used
- Have two or more active clients on the system [ClientA], [ClientB] and [ClientC]
- All three clients have data filed in table used in [WidgetA]
Steps
- Open the "Progress Note (Group and Individual)" form
- Select [ClientA] in the "Select Client" field
- Validate [WidgetA] reflects the data field for [ClientA], as expected
- Select [ClientB] in the "Select Client" field
- Validate [WidgetA] reflects the data field for [ClientB], as expected
- Populate all required and desired fields on the form
- Click [File Note]
- Validate the note files successfully
- Staying the form, select [ClientC] in the "Select Client" field
- Validate [WidgetA] reflects the data field for [ClientC], as expected
- Select [ClientB] in the "Select Client" field
- Validate [WidgetA] reflects the data field for [ClientB], as expected
- Select [ClientA] in the "Select Client" field
- Validate [WidgetA] reflects the data field for [ClientA], as expected
- Close the form
- At the home view, select [ClientA]
- Open [FormB]
- Validate [WidgetA] reflects the data field for [ClientA], as expected
- Make any desired changes and submit the form
- Validate the form files successfully
- Repeat step 2 for [ClientB] and [ClientC]

Specific Setup:

- Have an appointment scheduled for a client in form "Scheduling Calendar"
Steps
- Open the 'Form Designer' form.
- Select [Scheduling Calendar – Check In] from the 'Forms' field.
- Select the [Check In] section from the "Sections" field.
- Click [Show Section].
- Click [Add] in the 'Quick Links' section on the left side panel
- Search for a form in the "Search Forms" field. For this example "Update Client Data" form is used
- Select the form and click [Save]
- Click [Submit].
- Open form "Scheduling Calendar"
- Navigate to the appointment added in the set up
- Right-Click on the appointment on the appointment in the grid and choose "Check In"
- Validate the "Check In" form is launched successfully
- Populate and submit the form
- Validate the user is returned to the "Scheduling Calendar" form
- Close the form
- Validate the user is returned to the home view

Avatar eMAR - Filtering By Order Type
Scenario 1: Administration Events for 'Order Types' of "Pharmacy", "Dietary", and "Therapy" where 'Additional Attempts Planned' set to "No".
Specific Setup:
- Must have a value in the '(10000) Administration Event' dictionary under 'eMAR Tabled Files' in the 'Dictionary Update' form under 'Avatar CWS' that has 'Successful Administration' = "No", "Additional Attempts Planned" = "No", 'Limit To Specific Tabs' = no value and 'Add Reason Dictionary To Comments' = "Required", and 'Require Comments' = "Yes". (TEST)
- Must have a value entered in the '(10020) Event Reason' dictionary under 'eMAR Tabled Files' in the 'Dictionary Update' form under 'Avatar CWS'. (Patient Unavailable)
- Must have a value in the '(500) Order Type' dictionary under 'Order Entry Tabled Files' in the 'Dictionary Update' form under 'Avatar CWS' that has the '(501) Order Type Category' extended dictionary attribute set to "Pharmacy". (Pharmacy)
- Must have a value in the '(500) Order Type' dictionary under 'Order Entry Tabled Files' in the 'Dictionary Update' form under 'Avatar CWS' that has the '(501) Order Type Category' extended dictionary attribute set to "Dietary". (Dietary)
- Must have a value in the '(500) Order Type' dictionary under 'Order Entry Tabled Files' in the 'Dictionary Update' form under 'Avatar CWS' that has the '(501) Order Type Category' extended dictionary attribute set to "Therapy". (Therapy)
- The 'Avatar Order Entry->eMAR->Settings->->->Order Types Processed By eMAR' registry setting must include "Pharmacy", "Dietary", and "Therapy" order types.
- The 'Avatar eMAR->General->Settings->->->Enable Specimen Collection Events' registry setting must be set to "Y".
- The 'Avatar eMAR->General->Settings->->->Time Format Displayed in Avatar eMAR' registry setting must be set to "2".
- A "Pharmacy" order type code must exist. (Pharmacy Code A)
- A "Dietary" order type code must exist. (Dietary Code A)
- A "Therapy" order type code must exist. (Therapy Code A)
- Select the following order types from the 'Order Type' field in eMAR:
- "Pharmacy" in the 'Medications' tab.
- "Dietary" in the 'Nutrition Orders' tab.
- "Therapy" in the 'All Other Order Types' tab.
- Please log out and back in doing the configuration above.
- A client must have an active episode. (Client A)
- “Client A” must have a ‘Date of Birth’, ‘Sex’ and address on file in the ‘Update Client Data’ form, as well as information filed in the ‘Allergies and Hypersensitivities’ form, ‘Diagnosis’ form, and in the ‘Height’ and ‘Weight’ fields in the ‘Vitals Entry’ form.
Steps
- Select "Client A" and access the 'Order Entry Console'.
- Search for and select "Pharmacy Code A" from the 'New Order' field.
- Fill out any required fields and click [Add to Scratchpad].
- Search for and select "Dietary Code A" from the 'New Order' field.
- Fill out any required fields and click [Add to Scratchpad].
- Search for and select "Therapy Code A" from the 'New Order' field.
- Fill out any required fields, click [Add to Scratchpad] and [Sign].
- Validate the 'Order grid' contains the "Pharmacy" order, the "Dietary" order, and the "Therapy" order.
- Access the 'Avatar eMAR' widget.
- Remain on the 'Medications' tab.
- Validate that "Pharmacy" is still checked in the 'Order Type' checklist and that the order for "Pharmacy Code A" is displayed.
- Double click on any available administration cell under the current date.
- Verify the 'Order Acknowledgement' dialog is displayed and click [OK].
- Verify the 'Client Education' dialog exists.
- Select "Yes" from the 'Education Performed' field and click [OK].
- Set the 'Med ID' cell to the "NDC" of the order selected.
- Select "TEST" from the 'Administration Event' field.
- Click [View - Comment].
- Set the 'Reason' field to "Patient Unavailable".
- Set the 'Text' field to "test".
- Click [OK].
- If there are any warnings, populate the 'Override Reason' field.
- Click the 'Accept administration information entered' check box.
- Click [OK].
- Validate the cell selected under the current date contains "TEST" as well as "the time of the attempted administration".
- Uncheck "Pharmacy" from the 'Order Type' field and validate the order for "Pharmacy Code A" is no longer shown in the eMAR grid.
- Click the 'Nutrition Orders' tab.
- Validate that "Dietary" is still checked in the 'Order Type' checklist and that the order for "Dietary Code A" is displayed.
- Double click in any available cell under the current date.
- Verify the 'Order Acknowledgement' dialog is displayed and click [OK].
- Verify the 'Client Education' dialog exists.
- Select "Yes" from the 'Education Performed' field and click [OK].
- Set the 'Administration Event' field to "TEST".
- Click [View - Comment].
- Select "Patient Unavailable" from the 'Reason' field.
- Set the 'Text' field to "test".
- Click [OK].
- Click the 'Accept administration information entered' check box and click [OK].
- Validate that the cell that was selected contains "TEST" as well as "the time of the attempted administration".

User Defined Templates

Scenario 1: Validate data results using "User Defined " text templates
Specific Setup:
- The registry setting "Enable Templates on Demand for SQL Widget" is enabled.
- Have access to a form that contains as scrolling text field, for example a progress note form.
Steps
- Open the form
- Navigate to the scrolling free text field.
- "Right-Click" in the field (In "Avatar NX", click the widget template icon next field)
- Select "User Defined Template".
- Click "Manage Templates".
- Set the "Template Name" to a desired name [TemplateTest]
- Add fields such as Street, City, State, Zip by dragging from the right side panel
- Type in text in the field that also includes carriage return line feeds within the text
- Click the [Preview] button
- Validate the "Preview" window, displays all data as expected including the line feeds
- Click [OK]
- Click [Add]
- Validate [TemplateTest] displays in the bottom panel under "My Templates"
- Click [Save]
- "Right-Click" in the field
- Click "User Defined Templates"
- Select [TemplateTest] template
- Validate the field is populated with data and text based on the template set up in step 1
- Validate all data is formatted as expected, including any carriage return line feeds included in the template
- Submit the form
- Return to the form and select the row just submitted
- Navigate to the scrolling text field
- Validate the data populated and formatted in the field is as expected, including any carriage return line feeds included in the template

- The user has the "Forms & Data" widget on their home view.
- In the "My Forms" section, [FormA] has been added as a favorite
Steps
- Open form "Scheduling Calendar"
- Locate [ApptA] on the calendar grid
- Right-Click on the appointment
- From the drop-down list, select [FormA]
- Validate the form launches successfully
- Click the "(+) Browse forms" tab to the right of form name tab, to bring up the "My Forms" window
- From the "My Forms" list, click on [FormB] to launch it
- Populate the form and submit the form
- Validate the form files successfully
- Navigate back to [FormA]
- Populate any desired fields and submit the form

- A pharmacy-type order code with a prn 'Frequency Code' of "As Needed".
- A client must have an active episode.(Client A)

- Validate the 1st order's 'Start Date' contains the current date.
- Validate the 2nd order's 'Start Date' contains the current date.
- Validate the 3rd order's 'Start Date' contains the current date.
- Set the 'Order Group Start Date' field to a date that is five days in the future of the current date.
- Validate that the 'Start Date' fields for all orders contain a date that is five days in the future of the current date.
- Set the 'Order Group Start Date' field to a date that is five days in the past.
- Validate that the 'Start Date' fields for all orders contain a date that is five days in the past.
- Click [Add to Scratchpad].
- Validate that 3 orders are in the 'Scratchpad' with red flags.
- Select order 1 in the 'Scratchpad'.
- Populate all required fields and click [Update Order].
- Select order 2 in the 'Scratchpad'.
- Populate all required fields and click [Update Order].
- Select order 3 in the 'Scratchpad'.
- Populate all required fields and click [Update Order] and [Sign].
- If the 'Interactions' dialog is displayed, override all interactions and click [Save Override and Exit].
- Validate the 'Order grid' contains the 3 new orders.

Chart - Document output
Note - These testing guidelines assume the user is skilled in the use of, at a minimum, the following:
Scenario 1: Chart View - Validate "Print" and "Print All" Document Output
Specific Setup:
- Have a client [TestClient] with two or more documents created, for any form [TestForm]. For example, a progress note form
- For this example three documents are used [Document1], [Document2] and [Document3]
- Have the [TestForm] included in a user's chart view
- Log in as the user
Steps
- Select [TestClient] and right-click on the client to open their "Chart"
- Select [TestForm] from the "Chart Forms" list
- Select the episode tab
- Verify data for all three documents are listed consecutively, in the document display section
- Click the [Print All] link in the right corner of the tab
- In the print dialog screen, select "Microsoft Print to PDF" from the "Name" drop down list
- Click [OK]
- A "Save Print Output As" dialog will display. (Note user will be prompted with the "Save Print Output As" dialog for each row in the tab by descending data entry date/time order)
- In the navigation bar, navigate to a desired location to save all the files.
- In the "File Name" field enter the desired file name for [Document3]
- Click the [Save] button
- In the next "Save Print Output As" dialog, enter the desired file name for [Document2] in the "File Name" field,
- Click the [Save] button
- In the next "Save Print Output As" dialog, enter the desired file name for [Document1] in the "File Name" field,
- Click the [Save] button
- Open windows explorer and navigate to the location of the PDF documents saved in step 1
- Open the [Document1]
- Validate the contents of the page contains the expected data and data is in a readable format
- Open the [Document2]
- Validate the contents of the page contains the expected data and data is in a readable format
- Open the [Document3]
- Validate the contents of the page contains the expected data and data is in a readable format
- Navigate back to "Chart View"
- In the episode tab scroll to any one of the document rows listed in the tab, for example the row for [Document2]
- In the header section, click the "Print" link for that document
- In the print dialog screen, select "Microsoft Print to PDF" from the "Name" drop down list and click [OK]
- In the "Save Print Output As" dialog, select a location to save the file
- Set the "File Name" field to the desired file name [Document2]
- Click the [Save] button
- Open windows explorer and navigate to the location of the "Document2" PDF file
- Open the [Document2]
- Validate the contents of the page contains the expected data and the date is in a readable format

Treatment Plan - 'Assigned Services' grid
Scenario 1: Treatment Plan (Interventions) - "Assigned Services" grid functionality
Specific Setup:
- The client must be admitted to an active episode. (Client A).
- Registry setting 'Avatar CWS->Treatment Plan->->->->Enable Service Entry Restriction by Client Treatment Plan' must be enabled.
Steps
- Select "Client A" and access the 'Treatment Plan' form.
- Click [Add].
- Set the 'Plan Date' field to the current date.
- Select any value in the 'Plan Type' field.
- Select any value from the 'Problem List'.
- Enter any value in the 'Strengths' field.
- Enter any value in the 'Weakness' field.
- Enter any value in the 'Discharge Planning' field.
- Select "Draft" in the 'Draft/Final' field.
- Click [Launch Plan].
- Select the problem from the 'Tree View'.
- Select any value from the 'Status' field.
- Validate that all the fields display as expected.
- Add a 'Goal' and 'Objective' if desired and validate the fields display as expected.
- Click [Add New Intervention].
- Enter any value in the 'Intervention' text field.
- Select any value from the 'Status' field.
- Click [Add Service] in the 'Assigned Services' field.
- Validate the 'Assigned Services' gird fields are displayed in the following order: Service Program, Service Code, Frequency, Duration, Amount, Service Mode, Place of Service, Agency and Staff Responsible.
- Populate the 'Service Program' field.
- Enter a search value in the 'Service Code' field to bring up the listing of service code values.
- Select any service code.
- Validate that the service code field is populated as expected in the 'Service Code' field.
- Select any value in the 'Frequency' field.
- Select any value in the 'Duration' field.
- Enter any value in the 'Amount' field.
- Select any value in the 'Service Mode' field.
- Select any value in the 'Place of Service field.
- Enter and value in the 'Agency and Staff Responsible' field.
- Click [Add Service] in the 'Assigned Services' field.
- Populate all desired fields.
- Click [Return To Plan].
- Click [Submit].
- Validate the plan submit successfully.
- Select "Client A" and access the 'Treatment Plan' form.
- Click to edit the row just submitted.
- Click [Launch Plan].
- Click the 'Interventions' item on the plan tree.
- Click [Add New Intervention].
- Enter any value in the 'Intervention' text field.
- Select any value from the 'Status' field.
- In the 'Assigned Services' grid, click [Copy Service].
- Validate in the popup window, and check that the Amount field appears before Service Mode and Place of Service.
- In the 'Add Services From Other Interventions' dialog, choose the service added in the intervention previously submitted.
- Click [Copy].
- Validate that the 'Assigned Services' grid columns are populated with the service information, as expected.
- Click [Copy Service].
- Validate that the service previously copied is disabled and select the remaining service.
- Validate the services display as expected and neither are duplicated.
- Select the 'Assigned Services' row just added.
- Click the [Delete Service] button.
- Validate that the service row is removed from the 'Assigned Services' grid, as expected.
- Click [Return to Plan].
- Click [Submit].
- Validate the plan submit successfully.
Scenario 2: Treatment Plan Copy (Interventions) - "Assigned Services" grid functionality
Specific Setup:
- Client must be admitted to an active episode (Client A).
- A copy must exist of the 'Treatment Plan' form (Treatment Plan Copy).
- The 'Assign Services' section in the 'Program Maintenance' form has 'Enable Service Entry Restriction for Program' set to 'Yes' for one or more programs.
- The following registry setting must be enabled:
- 'Enable Service Entry Restriction by Client Treatment Plan'
- 'Activate Program/Service Code Filter'
Steps
- Select "Client A" and access the 'Treatment Plan Copy' form.
- Click [Add].
- Set the 'Plan Date' field to the current date.
- Select any value in the 'Plan Type' field.
- Select any value from the 'Problem List'.
- Select "Draft" in the 'Draft/Final' field.
- Click [Launch Plan].
- Select the problem from the 'Tree View'.
- Select any value from the 'Status' field.
- Validate that all the fields display as expected.
- Add a 'Goal' and 'Objective' if desired and validate the fields display as expected.
- Click [Add New Intervention].
- Validate the new column order is Service Program, Service Code, Frequency, Duration, Amount, Service Mode, Place of Service, Agency and Staff Responsible.
- Enter any value in the 'Intervention' text field.
- Select any value from the 'Status' field.
- Click [Add Service] in the 'Assigned Services' field.
- Validate the 'Assigned Services' gird fields are displayed in the following order: Service Program, Service Code, Frequency, Duration, Amount, Service Mode, Place of Service, Agency and Staff Responsible.

- Select the intervention and click [Copy].
- Validate that the 'Assigned Services' field displays the values from the previous intervention.
- Select the 'Assigned Services' row just added.
- Click the [Delete Service] button.
- Validate that the service row is removed from the 'Assigned Services' grid, as expected.
- Click [Return to Plan].
- Click [Submit].
- Validate the plan submit successfully.
