Improve OTO Frequency Logic backport
The following issues have been resolved: 1) The 'First Dose Date' field is populated with the correct value when "every nth day" frequency codes are selected. 2) One Time Only orders can be filed when the (524) Require Hours of Administration for Routine Orders extended attribute is set to "Not Required". 3) The 'Skip 1st Dose' button is disabled in the Order Entry Console (Orders This Episode section) when an order with a one time only Frequency Code is selected from an Order Group. 4) When the 'Require Hours of Administration for Routine Frequency Codes' extended attribute is set to "Require Hours of administration" or "Require Hours of Administration and enforce 'Times/24 HR' for pharmacy-type orders that any routine one time only order that is sent directly to the Scratchpad' will not receive a the message that 'Hours of Administration must be entered for "Routine" orders. 5) The Start Date/Time of an order is not effected by the First Dose Date/Time of an order when the First Dose Date/Time fields are disabled. 6) One Time Only orders that default to 'Give Initial Dose Now' can be filed when the '(524) Require Hours of Administration for Routine Orders' extended attribute is set to "Hours of Administration are Required...".

An issue is resolved in the Order Group dialog to ensure that when changing the 'First Dose Time' or 'Start Time' field of an order to a later time than defaulted; selecting or deselecting a different order in the group will not cause those fields to reset to the default times.

myAvatar - Orders This Episode - Order Groups with orders containing one-time-only Frequency codes
An issue is resolved in the Order Entry Console (Orders This Episode section) to ensure that changing the 'Start Date and Time' in the Order Group Dialog will update the 'First Dose Date/Time' of One-Time-Only orders within the Order Group.
The rules are as follows:
We will want to recalculate the 'First Dose Date/Time' based on the new 'Order Group Start Date/Time.
Scheduled One-Time-Only orders - will default to the next occurrence of the frequency from the newly adjusted 'Order Group Start Date/Time'.
Unscheduled One-Time-Only orders with no default hours of administration - the 'First Dose Date/Time' should be blank on initial order group load and when added to scratchpad, the 'Start Date/Time' for the order should equal the 'Order Group Start Date/Time' and the 'First Dose Date/Time' will be blank.
Unscheduled One-Time-Only with default Give Initial Dose Now - The 'First Dose Date/Time' will default to the next nearest hour/half-hour on initial order group load. If the 'Order Group Date/Time' or order group component 'First Dose Date/Time' is adjusted beyond the value set within the: "Avatar Order Entry->Facility Defaults->Client Profile->>>'First Dose' adjustment allowed when 'Give Initial Dose Now'" registry setting, the 'First Dose Date/Time' will clear and when added to scratchpad the 'Start Date/Time' of the order will equal the 'Order Group Start Date/Time'.
