Product Update Form Description

The system is enhanced to support fields SexualOrientationOther, GenderIdentity, GenderIdentityOther, MilitaryRelatedDisability, MilitaryBranchOfService, MilitaryServiceFrom and MilitaryServiceTo. This enhancement is specific to Cal-PM only and requires a Cal-PM update.

Avatar ProviderConnect filing of Avatar Cal-PM Member Demographic information to ProviderConnect is updated.
Avatar ProviderConnect is updated to support the following information fields in Avatar Cal-PM Member Demographics filing to ProviderConnect via the 'Update Client Data' and/or 'Admission' forms:
- 'Sexual Orientation'
- 'Sexual Orientation (please describe)'
- 'Gender Identity'
- 'Gender Identity (please describe)'
- 'Veteran'
- 'Military Related Disability'
- 'Military Branch Of Service'
- 'Military Service From'
- 'Military Service To'

Note, Avatar Cal-PM 2022 Update 80 and Netsmart ProviderConnect version 2023.2.1 are required for this updated functionality.

Avatar Cal-PM Member Demographic Information
Note - These testing guidelines assume the user is skilled in the use of, at a minimum, the following:
  • Admission
  • Demographic
  • Update Client Data
  • Look Up Client
Scenario 1: Avatar ProviderConnect - Verification of Client Demographic filing to ProviderConnect (Avatar Cal-PM System)
Specific Setup:
  • myAvatar system with Avatar Cal-PM parent product
  • Existing client record available to one or more ProviderConnect Contracting Provider agency/users
Steps
  1. Open Avatar Cal-PM 'Update Client Data' form (or 'Admission' form) for existing client.
  2. Enter or edit values for one or more Demographic data fields (Examples: 'Client Address', 'Date of Birth', 'Social Security Number').
  3. Enter/select value for 'Gender Identity' field (and enter value for 'Gender Identity (please describe)' field if applicable).
  4. Enter/select value for 'Sexual Orientation' field (and enter value for 'Sexual Orientation (please describe)' field if applicable).
  5. Enter/select values for one or more of the following Veteran information fields:
  6. 'Veteran'
  7. 'Military Related Disability'
  8. 'Military Branch Of Service'
  9. 'Military Service From' / 'Military Service To'
  10. Click 'Submit' button to file updated Client/Member Demographic data in Avatar Cal-PM and ProviderConnect.
  11. Log in to ProviderConnect system.
  12. In ProviderConnect Main Menu, open 'Lookup Client' form.
  13. Enter lookup criteria for desired client and click 'Search by Criteria' button.
  14. In 'Client Lookup' results, open client record for view.
  15. Navigate to 'Demographic' pre-display tab.
  16. In Client/Member Demographic record/data, ensure that values filed via Avatar Cal-PM are present in ProviderConnect system including the following fields:
  17. 'Gender Identity' (and 'Gender Identity (please describe)' if applicable)
  18. 'Sexual Orientation' (and 'Sexual Orientation (please describe)' if applicable)
  19. 'Veteran'
  20. 'Military Related Disability'
  21. 'Military Branch Of Service'
  22. 'Military Service From' / 'Military Service To'
Scenario 2: Avatar ProviderConnect - Verification of Client Demographic filing to ProviderConnect (Avatar PM System)
Specific Setup:
  • myAvatar system with Avatar PM parent product
  • Existing client record available to one or more ProviderConnect Contracting Provider agency/users
Steps
  1. Open Avatar PM 'Update Client Data' form (or 'Admission' form) for existing client.
  2. Enter or edit values for one or more Demographic data fields (Examples: 'Client Address', 'Date of Birth', 'Social Security Number').
  3. If using Avatar PM 'Admission' form, 'Veteran' (Yes/No) field may also be entered/filed under 'Other Client Data' section of 'Admission' form
  4. Click 'Submit' button to file updated Client/Member Demographic data in Avatar PM and ProviderConnect.
  5. Log in to ProviderConnect system.
  6. In ProviderConnect Main Menu, open 'Lookup Client' form.
  7. Enter lookup criteria for desired client and click 'Search by Criteria' button.
  8. In 'Client Lookup' results, open client record for view.
  9. Navigate to 'Demographic' pre-display tab.
  10. In Client/Member Demographic record/data, ensure that values filed via Avatar PM are present in ProviderConnect system (including 'Veteran' Yes/No display field value if filed in most recent Admission record).
