Product Update Form Description

The 'Move Selected Data', 'Product Final to Draft Override' and 'Document Routing Setup' forms are enhanced to support the 'Columbia Suicide Risk Assessment' form. Additionally, the 'Total Number of Preparatory Acts in Lifetime' and 'Total Number of Preparatory Acts in Past 3 Months' fields are added to the previously mentioned form.

Columbia Suicide Risk Assessment
The following issues have been resolved on the 'Columbia Suicide Risk Assessment' form: 1) Scrolling text fields were being truncated at 50 characters, 2) Data wasn't stored in the SQL table when certain not required fields were omitted. KB0068652 v0.01

Note - These testing guidelines assume the user is skilled in the use of, at a minimum, the following:
  • Client Alert dialog
  • Columbia Suicide Risk Assessment
  • Dynamic Form Mental Status Assessment
  • Product Final to Draft Override
  • Move Selected Data (CWS)
  • Clinical Document Viewer
  • Document Routing Setup (PM)
  • Dynamic Form - Document Routing Setup - Select Form
  • HomeView - My To Do's widget
Scenario 1: Validate Product Final to Draft Override
Steps
  1. Open a form such as the "Columbia Suicide Risk Assessment" form.
  2. Fill out all required fields and file the form in a "Final" "Status".
  3. Open the 'Product Final to Draft Override' form
  4. Set the 'Option' field to the desired form.
  5. Set the 'Entity Lookup' field to "Client A"
  6. Select any valid episode from the 'Episode Number' field
  7. Click [Select Row]
  8. Select any row
  9. Click [OK]
  10. Set the 'Override Reason' field to "Test"
  11. Click [Submit]
  12. Click [Yes]
  13. Verify the form files successfully.
  14. Open the assessment form opened previously in this scenario.
  15. Validate the assessment is back in a "Draft" status.
Scenario 2: Move Selected Data [CWS] - Moving Data from one episode to another
Steps
  1. Open a form such as the "Columbia Suicide Risk Assessment" form.
  2. Fill out all required fields and file the assessment either in a draft or finalized status.
  3. Open the "Move Selected Data" form.
  4. Move data from a client episode to another episode for the same client.
  5. Open the assessment form opened in step 1.
  6. Validate on the pre-display that the assessment was moved from one episode to another.
Scenario 3: Move Selected Data [CWS] - Move Core Form from one entity to another
Steps
  1. Open a form such as the "Columbia Suicide Risk Assessment" form.
  2. Fill out all required fields and file the assessment either in a draft or finalized status.
  3. Open the "Move Selected Data" form.
  4. Move data from a client to another.
  5. Open the assessment form opened in step 1.
  6. Validate on the pre-display that the assessment was moved from one client to another.
Scenario 4: Columbia Suicide Risk Assessment - document routing
Steps
  1. Open the "Document Routing Setup" form.
  2. Choose a form.
  3. Enable document routing for that form.
  4. Submit to file.
  5. Open the form that document routing is enabled for.
  6. Fill out all required fields and finalize the form.
  7. Sign the form.
  8. Open "Clinical Document Viewer".
  9. Locate the form that was finalized.
  10. Validate you can display and print the form.

Scenario 1: Columbia Suicide Risk Assessment - Field Validations
Specific Setup:
  • A client must be admitted to an active episode (Client A).
  • One or more clinical pathways defined in the 'Clinical Pathway Definition' form.
Steps
  1. Access the 'Columbia Suicide Risk Assessment' form.
  2. Select "Client A" and the desired episode.
  3. Set 'Assessment Date' to the current date.
  4. Select "Yes" in the 'Wish to be Dead: In Your Lifetime (When You Felt Most Suicidal)' field.
  5. Verify the 'In the Past Month?' field is required and select any value.
  6. Verify the 'If Yes, Describe field' is required and enter any value.
  7. Select "No" in the 'Non-Specific Active Suicidal Thoughts: In Your Lifetime (When You Felt Most Suicidal)' field.
  8. Verify the 'In the Past Month?' field is disabled.
  9. Verify the 'If Yes, Describe' field is disabled.
  10. Select any value in the 'Active Suicidal Ideation with Any Methods (Not Plan) Without Intent to Act: In Your Lifetime (When You Felt Most Suicidal)' field and validate the event logic works as expected.
  11. Populate any required and desired fields and validate the event logic works as expected.
  12. Select "Final" in the 'Status' field.
  13. Click [OK] on the 'Once set to 'Final', the data cannot be edited in the future' prompt.
  14. Select "Yes" in the 'Enroll in Clinical Pathway' field.
  15. Select desired pathway from the 'Pathway Name' field.
  16. Click [Submit].
Scenario 2: SQL table SYSTEM.Columbia_Assessment
Specific Setup:
  • A client must have the 'Columbia Suicide Risk Assessment' assessment filed (Client A).
Steps
  1. Access Crystal Reports or other SQL Reporting tool.
  2. Select the CWS namespace.
  3. Create a report using the 'SYSTEM.Columbia_Assessment' table.
  4. Validate a row is displayed for the assessment on file for "Client A".
  5. Validate all filed information is displayed.
  6. Close the report.
