The following changes are made:
1) Avatar CareFabric is changed to properly format names and include medical credentials for the 'Medical Director' property in the Flowsheet Application. Note: Avatar CWS 2022 Update 53 and Avatar CWS 2022 Update 63 are needed to support the full functionality of this update.
2) Avatar CareFabric is changed to display credentials description in the 'Flowsheet' and 'Mednote' applications.

The 'previousVisitIDByProviderID' field is added to the 'GetIntegratedCareVisitType' SDK action to allow a user pull forward the last completed document from a different author or their last completed document for Medical Note CarePOV.
