Record information for the Diagnosis section of the Adult Diagnostic Assessment Update.

  1. In the Diagnosis Update field, select whether there is a change in the client's diagnosis. Selecting Change Indicated enables diagnosis-related fields.

  2. For the Diagnosis Format field, select whether to use the DSM-IV or ICD-9 format.

  3. For the Diagnosis field, select the diagnosis. This field displays the episode date and diagnosis from myAvatar PM and includes the type of diagnosis and principal diagnosis code.

  4. Depending on the diagnosis format and the diagnosis selected, the Primary Diagnosis and Axis I, Axis II, and Axis III fields display applicable information. Update this information as needed.

  5. For the Axis IV fields, indicate if the client has a problem and enter a description of the problem.
