Enter information for the Conclusion section of the Lethality Assessment.

  1. In the Overall Risk Level - Self and Overall Risk Level - Others fields, select the choice that best describes the client's overall risk level.

  2. In the Comments field, enter comments about the client's risk level as applicable.

  3. For  the Provider and Provider Credentials fields, enter the name and credentials of the provider completing the assessment.

  4. Click Submit to file the assessment.
