Provider Fee Table Report

Generate a report that lists provider service fees (Provider Fee Definition form).

1. In the Individual Or All Providers field:

• Select All to generate a report for all providers.
• Select Individual to generate a report for a single provider.
  • In the Provider field, search for and select the specific provider.

2. Click Process.
