Care Manager Section

Enter Care Manager information.

  • This section of the Service Authorization form allows you to add or change the Care Manager that is assigned to the client. Note that the Care Manager is not tied directly to the authorization, it's an assignment directly to the client's record. The field is essentially a shortcut to the Assign Care Manager form. The Care Manager field will default the most recent Care Manager that had been assigned to the client in relation to the End Date of Authorization.
  • End Date of Authorization can also be added through the Budget Tracking Account Setup form.
  1. In the Care Manager Assigned field, enter the user name, and select. This field will automatically default a user where the End Date of Authorization falls after their
    Date Care Manager Assigned, but before the Date Care Manager Assigned of any other Care Manager.

  2. In the Date Care Manager Assigned field, enter the date the care manager was assigned to this member's record.

  3. In the Primary Level of Care field, select the level of care.
