The FoRSE Facility Configuration section of the FoRSE Configuration form enables an organization to create and record facility codes and view the FoRSE approval status of a new facility request from within myAvatar.

This configuration is used to create new facility codes and show their statuses in the system. All fields are required. These facilities are associated with the mapping (see the following table) and used when sending information to FoRSE.
The EHR Facility ID field is internally defined by the organization; it is not provided by FoRSE. The EHR Facility ID must be unique when more than one organization location or program may be configured. At least one EHR Facility must be defined; if necessary multiple facilities may be recorded. The mapping of assessment data to send to FoRSE may be unique by facility on the FoRSE Mapping form.

Test Facility	TestFacility	Indicate whether the data sent for the defined facility is test data or production data. Once set, this value may not be changed.
Enabled	enabled_code enabled_value	Disable to stop sending data for the defined facility.
