Outbound Referral History - PM
Document that an external outbound referral took place as well as view a list of referrals that were sent via the CareConnect Inbox.
- Go to: Avatar PM > Referral Management > Outbound Referral History
- In the Client Search field, search for and select the client by name or ID.
- The Referrals grid displays a list of all the outbound referrals for this client. Click New Row to add a new record.
- In the Receiving Organization field, enter the name of the organization who received the referral.
- In the Receiving Provider field, search for and select the provider who received the referral.
- In the Referring Provider field, search for and select the provider who sent the referral.
- In the Reason For Referral field, enter an explanation for why the referral was needed.
- In the Notes field, enter any additional information to accompany the referral record.
- In the Sent CCD Electronically, check Yes if the CCD file was sent electronically with this referral. This field is always marked as YES when receiving a referral from Inbox. Note: Now that we are processing referrals without a CCD, we need this to only populate a YES when we know a CCD is included. Sending referrals from CareConnect Inbox will NOT select the Sent CCD Electronically field in the Outbound Referral History form. This field updates appropriately and will only be set to Yes when a CCD is attached to the referral.
- In the Status field, select the status of the referral.
- Click Submit.
NOTE: If SDK is enabled, the system uses SDK action (a web service search) to search and populate fields such as Receiving Provider. If SDK is not enabled, the configuration in the Referral Management Configuration is submitted.
