Disclosure Management Configuration - PM

Define the configuration for disclosure packets generated by the Disclosure Management form.

You can add a logo to be added to the cover sheet of disclosure packets. This allows you to present a professional cover page on the packets provided to requesting organizations.

  1. Go to: Avatar PM > Client Management > Disclosure Management > Disclosure Management Configuration
  2. In the Logo field, right-click and select Acquire Image. This launches a navigation window, allowing you to select an image from your files. Currently, .jpg files are accepted.
  3. Select your image, and click Open. The image displays in the Logo field.
  4. To edit the size of your image, enter the Logo Height, and Logo Width in the corresponding fields.
  5. In the Watermark Text field, enter the text to appear as a watermark on disclosure packets. Watermarks appear on all pages of the document.
  6. In the Valid Attachment Types field, select the types of attachments allowed to be included in disclosure packets. This field includes all modeled forms as well as any product forms that allow inquiry printing.
  7. In the Re Disclosure Statement field, enter the disclosure statement for the document. 
  8. Click Submit.
