Outside Providers - Cal-PM

Maintain a record of providers outside of the facility that provide client services.

  • This form is for information-only and can be modified using the modeling functionality of RADplus Utilities.

  1. Go to: Avatar PM > Client Management > Client Information
  2. In the Select Client screen, enter the client name or ID in the Client Name/ID field, click the Search button, and select the corresponding entry.
    • If the client has multiple episodes, the Episode Information pre-display shows. Select the episode and click OK.
    • If the client has previous outside provider records, select the record. Click Edit to change information, or click Add to create a new record.
  3. In the Outside Provider Type field, select the type of outside provider. Enter outside provider demographic information as appropriate in the remaining fields.
  4. Click Submit.
